Question

I'm 21 weighing 121lbs, I've been thinking about a breast lift because its had quite an impact on my self confidence. Which procedure would be ideal for me with minimal scarring?

Hello @lifechoices22, thank you for your question. Yes, you are a candidate. You could consider a breast lift with BRALIFT (Breast Reconstruction Anatomical Lift) technique and it will for sure enhance the shape and projection of your breast. This procedure will remove excess skin, reposition breast tissue in order to create a better shape, symmetry and cleavage with both nipples lookint to the front. As a 23-year-old surgeon, I have modified the incision to ensure minimal scarring and the most appropriate result. Most incisions are inverted T-shaped scars. However, I have modified the scar to improve the extension of the scar into an L shape to ensure minimal scarring. I link a video to shed more light. Hi, thank you for sharing your question. You have some asymmetry between the breasts which can be corrected with a breast lift. A circumareolar lift will have the least amount of scarring.

my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity.

Thank you for your query. I would not recommend any scars at your age and  stage of breast ptosis, for a breast lift. The best option, in my opinion is a Thermi Breast lift using needles and also some autologous fat grafting through needles. This should give you the required improvement without any scars. You look like a great candidate for a mastopexy. No one wants scarring but with the proper after care, scarring can be kept to a minimum. There are so many ways to help scars heal smoother, and faster these days, and lasers to accelerate the fading and camouflaging of them. You would need a lift with pocket modification, to help your breasts become lifted and symmetrical. Along with the lift you could choose to add implants, or fat transfer to your breasts if you desired fuller breasts. Areola size could also be changed or kept the same during the procedure as well. If you are generally healthy and optimized for surgery, then it is possible to be a candidate for a breast lift. There are multiple breast lift techniques and each comes with it's own advantages and potential drawbacks. This is a good question. If you are happy with the size/volume of your breasts, a lollipop breast lift (circum-vertical mastopexy) would suit you best. If you want to be a bigger size, you may benefit from a breast augmentation, using your own fat or implants. Depending on the extent of your augmentation and skin characteristics, you may or may not need a lift at the same time. 
If you are looking to have children in the future, you may decide to wait until you are finished with pregnancies and breast feeding as those can change the size and shape of your breasts, ultimately leading to sagging breasts later on. You may still considering undergoing a breast lift now while planning to have children later - it is just important to know that you may need a second breast lift after child-rearing. Given your anatomy in the photos, you are a great candidate for a breast lift / mastopexy. Your nipples can be fully lifted, the breast shape rounded, and the breast appropriately positioned over the chest. The most important thing to consider is the scars that are required to achieve a satisfactory lift. At minimum a lollipop scar is required. This is a scar around the areola and straight down to the underfold of the breast. There is usually a short horizontal scar in the underfold of the breast in the midline. We perform the Awake Breast Lift® in the office under local anesthesia and oral sedation.

I recommend an in person consultation with a plastic surgeon to discuss your goals and be measured. After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. You get to see actual results and the surgeon's experience and not a virtual idea of what can be achieved. Women tell me that this process is very helpful in determining what size and style breast implant is right for them. Mildly Sagging Breasts: If your breasts are mildly droopy, a saline or silicone gel implant placed behind the pectoral muscle might help create the illusion of perkiness. That’s because as the implant fills out the top of the breast, it also fills out the bottom, making it look as if the nipple has moved higher. It hasn’t.
