Unfortunately, one of the drawbacks of fat transfer to the buttocks is the potential worsening of cellulite. Cellulite is completely normal and everyone has it. Of course, some people more than others. What makes the dimples are fibers which are part of the connective tissue. These fibers connect the skin to the muscle fascia and are not very flexible. The fat is located within this space. As the fat volume increases (weight gain or fat transfer), it causes those fibers to stretch, making dimples more noticeable. On the other hand, decreasing the fat volume will make the dimples less noticeable. This of course defeats the purpose of the fat grafting. However, these are the compromises we have to make. Looking at your photos, I can see what you are concerned about, however I think you have a good result and have a great shape of your buttocks. The appearance of the cellulite is normal and acceptable. Thanks for your question and good luck!
It is hard to say what this is and more difficult without preoperative photos. Lumpiness and bumpiness are common after fat transfers and liposuction but if you have any concerns you should always see your plastic surgeon right away as some complications (like infection - often manifested with pain, redness, fever) can be treated if caught early enough.
