I am posting current pics and wish boob pics. I've had 2 consults and one recommended 400-450 max HP w a lollipop lift and the other recommended 450-560 HP/XHP round with no lift. Both recommend under the muscle gummy bear silicone gel (inspira). Which profile would ki recommend? Lift or no lift? How many cc to get close to the look I am wanting?

Hello and thank you for your question. You are a great candidate
for a breast augmentation using a dual plane technique without need for a formal lift.  The size, profile, and shape of the
implant is based on your desired breast size/shape, your chest wall
measurements, and soft tissue quality.  This decision should
be based on a detailed discussion with equal input from both you
and your surgeon.  This entire surgery
can be performed with a small incision technique.  Make sure you
specifically look at before and after pictures of real patients who have
had this surgery performed by your surgeon and evaluate their results.   The most important aspect is to find a
surgeon you are comfortable with. I recommend that you seek consultation with a
qualified board-certified plastic surgeon who can evaluate you in person.

Implant profile is a very important consideration. Many patients wrongly assume that the higher the profile the better. The truth is, the more profile (or projection/height) and implant has, the narrower its base has to become. This is associated with several problematic consequences. 
  
The first issue is that the patient loses width and therefore cleavage of the breast. The second is the breast becomes more narrow and pendulous, often falling lateral and onto the side of the chest, and third, high profile implants have a higher risk of bottoming out because they exert more pressure on the inframammary fold than moderate profile implants. 
  
High profile implants do have a role in symmastia repair (when I need to narrow the base width) but otherwise I try to avoid them.
  
Also, determining whether you need simply a breast implant versus a breast lift or both can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. 
  
Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just breast augmentation depending on the size of the implant the patient wants. I often use an implant when I perform a breast lift because it provides more upper breast fullness after the lift. Also, price range for breast lift is around 10k to 15k depending on whether you need a lollipop or a donut mastopexy.
  
Another important consideration to make would be the breast implant type and sizing selection. In fact, this is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient.  
  
When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements. 
  
Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match. 
  
Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.
  
You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

Based on your photos, I agree that you would benefit from breast lift with implants. However, the photos you provide as the ones you like are all of primary breast augmentation without breast lift.
I would caution using implants that are too large (side boob) when performing a lift with implants. Scaring could be unacceptable and risk for problems is increased. Adjusting your expectations and finding pictures of breast augmentation with lift is a good start.

There are several variables to be considered when answering this question. It is always difficult to make a diagnosis on photos alone, but you do appear to be a good candidate for a breast lift with implants. When you meet with a board certified plastic surgeon, they will perform a set of measurements that will help determine whether or not a lift is necessary. Another variable that needs to be discussed is what you are wanting. If you are wanting a fuller upper pole to your breast, then an implant will be necessary.  If you are just wanting the breasts to be a little perkier and don't mind the size, then only a lift is necessary. And to have the nipple in proper position over the implant, may require a lift. Choosing the right implant for you require an exam in person.  As far as a lift is concerned, it looks like your nipple needs to come up a bit and the lower pole of gland may need to be lifted.

From the front, I would be border-line on the lift.  From the side, I would say you don't need it.  I would need to examine you with proper posture and see the quality of the tissues.  Your measurements sound almost perfect.  Implant needs to be placed through the fold and not be any wider or narrower than 12.5-13.5 cm in width (I would err right on the 13 cm side).  Extra full is great when the patient has no breast tissue of their own- you do, so I don't think you need that high of a profile.  I think a full and even a moderate would give you a gorgeous result.  Look at before and afters and read reviews.  Make sure you find the perfect surgeon for you!  Hope this helps.-JGH

Hello and thank you for the question and photos. Your goal pics all have a high profile look with a full d / dd cup size. The size you mentioned all sound about right for the desired final size you want to achieve. Based on the photos and the dimensions, I would be surprised if you needed a lift. Your breast has good nipple position and some volume loss which looks post pregnancy. Implants will restore this ( like adding air to a flat car tire ) and will help give a small degree of lift. If you are very athletic, I would recommend a textured silicone implant and place it in the dual plane / under muscle position. The surgery is done under anesthesia and will take about an hour. You should factor a week off of work and about a month out of the gym for recovery.
You should have several consultations with board certified plastic surgeons before making any final decisions. 
As always, best to be healthy, no smoking, and make sure any health concerns you have are managed by your primary care md.

Looking at your pictures you are most likely benefit from both a breast lift as well as breast implants. To figure out the best size for you I would utilize 3-D simulation, have you try on implants, and evaluate your body measurements to ensure the proper implant will fit you appropriately.
