Surgeon cleaned under GA & told me to inject under skin into breast pocket with iodine every 2days & dress with aqualcel ag. No infection but microfilm. After 3 treatments my breast has become hard, hot, sore and inflamed not at wound site but breast pocket. Allergy iodine? Infection? or cos i used waterproof sticky dressing instead of gauze doc told me said it needed to drain. Why is this happening.I just want to heal and be well?
Answer: Delayed wound healing following a breast augmentation/lift. Is the wound in the incision of the breast lift? Delayed wound healing at this location may be do to decreased perfusion, tension, or suture reaction. The redness, tendernes and heat maybe do to infection. The concern is an infection around the implant with possible implant exposure. A trial of antibiotic and even consideration for hyperbaric oxygen should be tried. If this fails it may be necessary to remove the implant, allow the wound to heal then replace the implant.

Answer: Iodine after breast lift and augmenation In my opinion, from your photos it appears at though you have an infected breast implant/pocket with a hypergranulated wound tract into the pocket. In most situations like this the implant is removed and the pocket washed out, a drain is placed and the infection is treated with antibiotics and allowed to resolve. This situation will not usually "heal" with local wound care. I would recommend seeking another consultation with at least one other board certified plastic surgeon in your area to obtain advice and treatment options. Answer: Hot, hard, sore breasts with inflammation 3 months post-op are infected until proven otherwise It is impossible to fully assess your situation long distance like this without an exam, but based upon your story and the images you have provided, my number one concern is that your implant and capsule are infected, and the implant will need to be removed until the infection can be cleared. Irrigating the pocket with iodine solution at home, if in fact you were actually irrigating the pocket around the implant, would almost certainly be expected to result in infection of the closed space around the implant at some point, in my opinion. Once this happens, you really have no recourse other than removal of the implant, as you will never totally eradicate the bacteria until you do. I have seen numerous attempts at salvage of implants like this, with patients only to have chronic recurring bouts of inflammation temporarily "cooled down" with antibiotics, chronic draining sinuses, and open wounds, only to finally undergo explantation in the end. Based upon the time course and the apparent redness of your tissues in these images, you should NOT delay in contacting your surgeon for evaluation. If you aren't seen within the next couple of days, something is amiss. Best of luck.

The medial breast appears quite red and I would be worried that you might have an infection in that area (if this is a new finding).

Answer: Management of Exposed Implant Requires Removal kerrina66,You appear to have an implant pocket infection/germ colonization and thus exposure of the implant has occurred within the depths of the open wound, which is obvious if you can irrigate around the implant in the pocket. That being the case, this will not likely go on to satisfactory resolution, with capsule contracture a virtual certainty, even if the apparent infection can be controlled locally. A study by Massachusetts Plastic Surgeon Eugene Courtiss, MD, published in the Plastic and Reconstructive Surgery Journal in the late 1960s or early 1970s reviewed the issue of implant exposure and outcome if the implants could be salvaged by antibiotics and wound treatment, and the outcome was uniform occurrence of capsule contracture, leading to unsatisfactory aesthetic outcome in those patients where the implants could be salvaged without removal. If there were no infection/inflammation, there would be no spreading redness and tenderness, but it may be difficult to culture germs if you are using Betadine, whether or not you are on an antibiotic. If you were to see me for consultation, I would recommend removal of the involved implant, which will allow rapid healing and then replacement in about 6 months, once the acute healing has occurred and the scar has softened. The gause and Betadine (povidone iodine) have nothing to do with the redness
