I had breast augmentation 3 weeks and 2 days ago. My right breast appears significantly larger still and my nipples look uneven. Is this okay at this point? (I realize that I did not have perfect symmetry prior to surgery and will account for that.)

At three weeks post-op, you are in the earliest stages of healing. Your breasts are still swollen and the implants have not had time to settle in the breast pocket. It will take four to six months for the swelling to completely resolve, sometimes longer, so it is hard to make a full assessment about your results until that time. It is common for one side of your chest to settle faster than the other, although this asymmetry will resolve over time. Additionally, if you had some degree of chest asymmetry before your BA, the surgery may make this more pronounced. I recommend being patient while you give your chest time to heal. If you have any further concerns, reach out to your operating surgeon.

Though this may be differences in your natural breast tissue +/- asymmetrical swelling from surgery, I would reach out to your surgeon to ensure more urgent matters like a fluid collection has not developed. Answer: Be patient! Thank you for sharing your photos. Your early result looks quite normal. Your implants are in the typical early position (relatively high on the chest wall) and will naturally migrate downwards as your tissues stretch and the swelling resolves. The relative position of your nipples will also improve as this occurs. The minor asymmetry is not at all uncommon (especially if it was present preop).You should remain patient, discuss your concerns with your plastic surgeon and he/she will probably also encourage you to perform some gentle massage/stretching exercises to move your implants downward and medially.
