Goal;SOFT full B/small C. I'm finally getting boobs & need implant advice! I'm 5’7”, 106lbs, bony chest & little breast tissue. I like the size in my goal photos & don't want to look vulgar or with 2 mounds stuck to my chest & a huge gap. Dr A: BWD = 11.7 → Natrelle 240cc low profile max. Dr B: BWD = 10.5 → Inspira 240cc medium profile. Anything larger would cause rippling. 1. Why is my BWD so different? 2. Which implant is best? 3. Will 240cc bring me to my goal photos? Based on your photographs, I think that you are starting at a good place and should have a very nice outcome with breast augmentation surgery. Ultimately, careful communication of your goals (in my practice I prefer the use of goal pictures, direct examination/communication in front of a full-length mirror, and computer imaging) will be critical. Generally speaking, the best online advice I can give to ladies who are considering breast augmentation surgery ( regarding breast implant size/profile selection) is: 1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. Ask to see lots of examples of his/her work. 2. Have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining breast implant size/type/profile will most likely help achieve your goals. In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. For example, I have found that the use of know words such as “natural” or "C or D cup” etc means different things to different people and therefore prove unhelpful. Also, as you know, cup size varies depending on him who makes the bra; therefore, discussing desired cup size may also be inaccurate. Again, the use of computer imaging has been very helpful during the communication process, in our practice. 3. Once you feel you have communicated your goals clearly, allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals. Again, in my practice, this decision is usually made during surgery, after the use of temporary intraoperative sizers. Answer: Natrelle implants comparison I work with the style 10 (low profile) implants and Inspira moderate profile implants quite often. For you, I personally like the Inspira route. You are so thin and fit with very little breast tissue and that implant produces less rippling than the style 10. I agree with both physicians that you shouldn't try to go much larger. It is important to get a good result with the first surgery. If the size isn't quite as big as you'd like, it will be easy to go back and replace with a larger implant in the future. Best of luck!

You are so thin and fit with very little breast tissue and that implant produces less rippling than the style 10. I agree with both physicians that you shouldn't try to go much larger. I prefer using the Cohesive Gel - Silicón Breast Implants with textured cover ("gummy bear implants") They are quite safe and aesthetically best as they give a firmer consistency, better projection and natural appearance.To place the implants, I recommend making the incision through the areola (leaving an unnoticed scar) in conjunction with a Round Block Areolar Reduction to get an appropiate areolar diameter, with a minimal and hidden scar (a circumference line surrounding the areola).The breast feeding chances can be affected to a lesser or greater proportion, depending of the surgical manipulation made in the mammary gland and areolas during the procedure. In my practice, I try to be as delicate as possible, preserving the integrity of the gland and areolas. Answer: Choosing the right sized breast implants I appreciate your concern, because size does matter when seeking a natural looking result in breast augmentation. I ask my patients to bring in photos of breasts the size they desire, because there can otherwise be confusion because B or C cup can vary greatly by brassiere manufactures! I also utilize the implant in the bra technique, with several different implant sizes with a tighter fitting T-shirt to get an idea of what you will look like. Finally, I prefer to meet with my breast implant patients at least twice before surgery, so I am confident that I know exactly what her desires regarding shape and size are. I then have several sized implants in the patient's desired range available in the OR to allow me to tailor the operation to her needs. Be certain your surgeon understands your desires and you should do great as this operation carries a very high satisfaction rate. Good luck!

Answer: Natural breasts in the slender patient. Thanks for your question and the photos. There are always trade offs in a slender patient such as yourself. When there is little natural breast tissue implants of any kind are less well hidden. In my approach to the slender patient I recommend three things:Common sense size.Under the muscle.Silicone.For your concern on base width I suggest you have your ps have you try on implant sizers. A gentle curve under the arm which balances with the curve of the hip is what I look for.For your concern about cleavage, most plastic surgeons will place implants as close to the midline as possible, but how close they get in a submuscular placement will depend upon the insertion of your muscles. We do not like to elevate the sternal insertion of the pec muscle as this can cause synmastia or breadloafing.With respect to the size, your goal images are between a low and moderate profile, again , trying on sizers can help you with this decision. You may also want to consider a Mentor cohesive implant. I find the shell is softer and may look more natural in the slender patient.I hope this helps and I have included a sizing link as well.
