Small, pointy boobs- Want to fix shape without adding much size. Donut lift w/implants? Constructed lower pole release?

Question

I am 5'10 155lb with an athletic build. Currently, my breasts look like this, and I really want to change the shape so that they appear more round and developed. However, I want to keep them small as I really prefer the look of small boobs. I also want my nipple to be more centered instead of pointing down. I'm considering a donut lift with a small implant- would you recommend this? Also, does it appear that I have lower pole constriction that needs releasing? I've attached current and goal pics

Thank you for your question.  I think you are a great candidate for a breast augmentation with the implant placed under the muscle.  You do not have to go big in size, you can do a small implant to just help with shape and it will help rotate the nipple up a bit. I would suggest seeing a Board Certified Plastic Surgeon in person to evaluate you and discuss your options with you.

I'm not sure the lift mentioned would be super beneficial. I think this case is more about lowering the fold and centering the implant behind the nipple which should make it appear to point outward instead of slightly down.

Based upon measurements of your chest wall and breast imprint width as well as knowledge of all breast implant sizes and profiles should make this process easy. Try on a profile bra with various silicone sizers your PS recommends under a tailored blouse, tank top, bikini, workout clothes, and other garments you typically wear so that you have the best idea of whether the garments fit as desired with the implant selected. Select the size that boosts your self-esteem, and fits your anatomy. Choosing an implant based on an aesthetic goal that exceeds your anatomic capacity may lead to complications and possibly revision later in life.

There are a number of options to enhance the contours of your breasts and to center the nipple position on the breast mound. The procedure of choice would depend on your goals and what outcomes you would accept. Seek a consultation with a board certified plastic surgeon who can guide you through the process to select the best option for you.

The technique I recommend is a modified Benelli lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, fat transfers can be performed.

Given the photos, I would likely approach your procedure with placement of implants and lowering the fold, without needing a donut lift.  Judging by your photos, that would be the most sound approach and would give you a much better result.
As far as sizing goes, we perform in office sizing, so there is no second guessing as to how big or small you would want to be.

I think you are better off with standard breast augmentation and lowering the fold a bit. Donut lift will do nothing except distort the areolae and give you widened scars.

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

Just based on your photos, an augmentation should be able to lower your fold and give the appearance that your nipple is centered. I don't think you need an actual lift. However, I recommend meeting with a board certified plastic surgeon and discussing your options.

Thank you for the question and photos and you might benefit from release of the lower pole with implants alone and may not need a lift.  So go on some complimentary consultations with experts and compare recommendations after a full examination and discussion of desired results and look at photos of similar patients and become fully informed about the desired procedure
