Is there anything else that can be done? I hate how it looks the tip is totally uneven. He said the skin is too think and stuck to scar tissue so too risky to try. I have been reading about steroid injections could they help me? He also said it's likely it will keep changing and getting worse. I am devastated

Thank you for the question and the photos. While you have some asymmetry, it doesn't look like the issues are overwhelming. A conservative revision could help with the top shape, but must be balanced against the risk of poor healing and new scar formation. That said, even in patients with very thin skin, nasal rip surgery is possible. In my practice, I use temporalis fascia grafts to cover the tip cartilages and provide camouflage. There are some minor issues in the nasal tip, however from the one very limited photograph, there significant asymmetries in the bridge line. The nasal bones can be narrowed and straightened with osteotomies since the bridge line is overly widened and asymmetrical.  Placement of steroid shots in the nasal tip will only make  the skin  thinner.  Revision rhinoplasty is a very difficult endeavor, so choose your surgeon wisely based on extensive experience

In my opinion, based on your photos, this looks like a result that can be improved with a revision surgery.  There are ways to "thicken" the skin envelope and eliminate the distracting "surgical" appearance of the tip but it is imperative to consult with 2-3 surgeons who are experienced in revision rhinoplasty. Best wishes!  Harry V .Wright MD, Sarasota, Florida

You need an in-office exam to determine this.  From the photos, it appears that a revision might be possible.  You won't be able to get rid of all the irregularities, but most of them could be improved.  Factors that can influence the decision to operate are the skin thickness, turgor (how much edema is left from prior surgeries), how long after the last surgery, medical side issues, risk to benefit ratio.  Seek out a board-certified Facial Plastic surgeon for a consult.  Best wishes.

Any revision surgery carries risks of unexpected or unwanted results due to scar tissue from the original surgery.  Many of these risks diminish over time as scar tissue softens.  In rare cases, I have encouraged patients to wait 2 or 3 years from the original surgery to allow complete deep healing.  In your case, I would seek more than one opinion.  Look for a very experienced rhinoplasty surgeon.  Sometimes accepting a small, but visible scar is worth the overall improvement you seek, so ask the surgeon to be clear about risks and benefits
