30 year old Polyurethane Silicone above muscle. 32aa now a large 32c. One would remove in one piece, due to calcifications and thin skin might have to leave some of the silicone if can't get it out around the calcification and would also put some stitches inside by my nipples to make sure my nipples stay in the right place while healing, said no lift req. 2nd opinion said try to remove them in one piece but might not be able to and would just flush the pocket out if silicone leaks.
Answer: En Block Capsulectomy Generally, an en block capsulectomy is best for old ruptured and calcified implants. In this procedure, this often commonly fold into not thinking that you need a left as the capsules actually holding up periareolar tissues. I have seen this on many occasions which a capsulectomy is performed and then the surgeon is surprised to see that the patient actually did need a lift. Another thing to consider in many of these cases is a re-positioning of implant into the subcutaneous pectoral location. Make sure that you see a plastic surgeon who is very experienced in revisionary breast surgery. Best wishes to you.

Answer: Trust Trust in your surgeon to remove all aspects which need to be removed. I recommend an in-office examination as well as a detailed discussion with a surgeon who you are comfortable with. Finally, make sure your surgeon is a Double-Board Certified Plastic Surgeon certified by the American Board of Plastic Surgery (ABPS), a member of the American Society of Plastic Surgery (ASPS), a member of the American Society of Aesthetic Plastic Surgery (ASAPS), and a fellow of the American College of Surgeons (FACS).Best,Dr. DesaiBeverly Hills Institute for Aesthetic Plastic SurgeryHarvard Educated, Beverly Hills & Miami Beach TrainedDouble Board-Certified Beverly Hills Plastic Surgeon

Answer: En bloc capsulectomy An en bloc capsulectomy is typically performed in the presence of older ruptured silicone devices. Due to the viscous (think "honey") nature of the silicone fill, removing the entire capsule at once (without exposing the ruptured internal contents) is the most practical approach to ensuring complete removal. It is important to remember that while the "goal" is complete removal (capsule and contents) there is always the potential for some retained capsule or silicone. In most cases, the vast majority of the silicone/capsule will be removed and any residual is unlikely to cause an issue. As always, discuss your concerns with a board certified plastic surgeon (ABPS).

Answer: Do I need to ask for an Enbloc procedure only due to my old hard calcified silicone? Thank you for the question. Based on your description, it I would likely recommend en bloc removal of the breast implant. En bloc breast implant removal refers to a procedure where the breast implant and the surrounding capsule is removed as a single unit. The procedure is performed such that the contents within the breast implant capsule do not come into contact with the surrounding tissues. The procedure may be indicated when a patient has ruptured silicone gel breast implant (especially older silicone gel breast implants), or if the patient has concerns about "medical conditions" related to the breast implants. Best wishes.
