1.5 years post deep plane facelift - Can this area on my chin be revised though the existing scar? (Photos)

Question

I had a deep plane facelift 1.5 years ago and due to complications I have a bulge/double chin. My doctor suggested I have a revision facelift but I don’t want to go through the whole process of scarring and healing again. Do you think it might be possible to revise it through the existing scar under my chin?

Dear Cj2475, Thank you for your question. Amongst a number of features evident, you appear to have an asymmetrical submental scar, bulky submental soft tissue, platysma bands and loose neck skin. Whilst I do not know the scope of the surgery you underwent, it may be possible to address some of the matters identified through isolated submental access, but you ought be cautious about the interplay of any associated result with the jowls above to avoid a dysjunction effect. That is likely how the suggestion of revision facelift has arisen, which may indeed be most appropriate to redrape your tissues optimally. Much more information is needed such as your age, skin tone in the neck, and a copy of your operative report to find out what was done in the first procedure. From these limited photographs, you have fat deposits in your neck which could be located both above and below the platysma, and anterior platysma muscle cords. As long as your skin tone is acceptable, a neck lift only can be performed through a 1 inch incision underneath the chin.

Thank you for your question.  I have been in practice for over 30 years.  Yours is a very simple and easy fix with liposuction, fat equalization, and some fat transfer.  You should be able to have a beautiful result with a minimal procedure.  Best wishes to you.

Hello. A persistent submental bulge 1.5 years after a deep plane facelift is usually residual fat, incomplete platysma work, or both. Revision through the existing scar under the chin can help if the problem is only the central neck. If there is also residual jowling or skin laxity, a limited submental approach will not be enough, and a revision deep plane face and neck lift is the better option. I would need to examine the neck in person to see which applies.
