I had a deep plane and neck lift almost 5 years ago. What procedures would you recommend to get rid of loose skin/ pouch under my chin? I am 61 yo and 130 pounds

Thank you for your question and photos. 
I think you have a range of options based on how you would like to proceed. One option would be to discuss whether any of the non-surgical skin tightening devices (ie radiofrequency, deep laser resurfacing, etc) may be an option. The other option would likely be a revision neck lift with focus on addressing the excess skin/platysma muscle in the area and re-lifting into a better position. I would also discuss with your primary surgeon as they may have a better understanding of why you have had relapse in this area and know best options for you.

Thank you for sharing! Since you’ve already had a deep plane facelift and neck lift, I would really need to evaluate you in person to determine exactly what is contributing to the fullness or pouch under the chin. In that area, it can be a combination of loose skin, fat, and/or the underlying neck muscles, and the treatment depends on which of those is causing the concern.
Generally, when there is recurrent skin laxity several years after a facelift/neck lift, achieving a meaningful improvement often requires some type of revision procedure to adequately tighten and support the skin again. Once I examine the area, I can give you a much better idea of what would give you the best result.

First of all, this is partially related to neck work and the way surgeons do deep plane facelifts. You may benefit from a revision to the area though it can be difficult.  Secondly, and perhaps better news, is that some looseness in this area is common after surgery. We now have a technology called Junera which uses a laser under the skin to tighten these areas quickly and easily in the office. The technology is quite new, but seems very effective.
