Your doctor is correct in saying that there is only so much fat to work with.  That is a correct statement if the source is your lower eyelid fat.  It is difficult to tell without photos, but at this point, your options are to use a filler or another fat graft from a different area.  Best of luck.

My question would be whether your doctor also did fat transfer at the time of the bleph.  If that is the case, the swelling takes longer to resolve.  Be sure to follow with your doctor.

This is a small opening on the suture line and it should heal well with more time. At this point, I would not suture this dehiscence and let it heal in the natural course.
