Hi MszDiamond, Determining whether you need simply a breast implant versus a breast lift or both can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just breast augmentation depending on the size of the implant the patient wants. I often use an implant when I perform a breast lift because it provides more upper breast fullness after the lift. Also, price range for breast lift is around 10k to 15k depending on whether you need a lollipop or a donut mastopexy. Another important consideration to make would be the breast implant type and sizing selection. In fact, this is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements. Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match. Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small. Having not seen you, I would still guess that you most likely need a breast augmentation AND a breast lift (mastopexy). The augmentation will increase your cup size, namely going from the D to DD-cup. The mastopexy will address the saggy appearance. These procedures can and are often done at the same time. Combining the two is helpful from a cost reduction and total recovery time standpoint. I would advise meeting with a board certified plastic surgeon so that you can discuss your goals. Having a physical exam will be most helpful for the surgeon to determine optimal implant size and type of lift, if any, that is indicated. Best of luck!

Without photos it is difficult to give feedback, however a patient who presents with "saggy" breasts are typically candidates for a mastopexy in conjunction with implants for added fullness in the upper pole. I recommend discussing these options with your board certified plastic surgeon, who can individualize your treatment plan.
Hi. If you have sagging then a lift with implants is going to give you the best shaping. Placing an implant, even an overinflated saline implant, will only expand the upper portion of the breast, and will not reposition the nipple to be centered. The looser your tissues (from weight gain, nursing, aging, etc) the more likely you need a lift. The use of a high projection implant will help give you cleavage and increase cup size, signs of perkiness. Have an in person examination and consult on the different implant options with an ABPS certified PS. Good luck!
There is nothing usually perky about a DD cup size as these are quite large and usually have some ptosis associated with them. If you would like perky, then a lift without implants would probably be a better plan.

With thatbeing said, based on your description, the best shape and contour could likely beachieved with an implant and a breast lift. A breast lift with an implant can result in a verynice shape if good technique is used. The size, profile, and shape ofthe implant is based on your desired breast size/shape, your chest wallmeasurements, and soft tissue quality. This decision shouldbe based on a detailed discussion with equal input from both youand your surgeon. Make sure you specifically lookat before and after pictures of real patients who have had this surgeryperformed by your surgeon and evaluate their results. Hello dear, thanks for your question.The breasts may lose their elasticity and firmness which can be caused by different factors such as pregnancy, massive weight loss, lactation and aging. To reaffirm the breasts and restore the natural look your surgeon can perform a mastopexy or breast lift with augmentation such as silicone implants, you will get very nice results, contact a board certified plastic surgeon, I recommend you soft cohesive silicone implants about 350 cc's you will look pretty and they will be perky.Patients who goes to a consultation for drooping breasts (ptosis) or volume loss after pregnancy (pseudoptosis) almost always have one very important question. "Can my drooping be corrected with just implants or do I need a lift".Implants alone will correct drooping when:Breast drooping is mildThe nipple is still near the center breast and does not point downwardThere is some visible skin beneath the nipple/areola when looking at the breasts straight on.The patient is okay with being at least a cup size largerA breast lift (with or without an implant) is better when:Breast drooping is moderate or severeThe nipple is at the bottom of the breast or points downwardThere is no visible skin under the nipple/areola when looking at the breasts straight on.The patient is already a C or D cup breast size and doesn't want to be largerThe areola is too large and the patient wants it reducedAlso show the patient photographs of a spectrum of breast scar present after a breast lift in order to check their tolerance for this scar. Most patients are surprised how little the scar shows and are okay with proceeding. If the patient really needs a breast lift, but is concerned about the scars, I will suggest they wait on surgery. In time the breast appearance problems will usually outweigh their concern about the scars.I use implants in combination with a lift when:The patient wants to be larger in addition to being less droopyThe patient desires to maintain projection and roundness of the breast moundThe drooping is severe.....
