How can I have the dents and adhesions from my breast explant corrected? (photos)

I had an explant of my Allergan textured implants 5 weeks ago. I have been doing daily massage. It seems I have a point of adhesion on both breasts right under my areola. When I push up on my breasts from the bottom this area sticks down, I have included pictures to show the dents caused by the area that is sticking or adhering down. Can this be corrected and what is the recommendation for the correction?

Five weeks is still early, it can take several months for the tissue to settle down. Revisions too soon with dense areas will limit results. After time, the areas are likely to soften and smooth out on their own. Otherwise, options can be discussed which may include a combination of scar tissue removal, fat transfer, new implants, or no surgery.Close follow up with your plastic surgeon is recommended during this time for instructions.

Thank you for your question and for your photos. Based on your photos provided, this type of event is sometimes common. I would advise you to continue to massage the breasts for further improvement. Please note that the implants are removed and it will take months to adjust in place.

Five weeks after explantation, there are still changes occuring. Vigorous massage may still be of benefit. I also have had success in correcting similar problems with the use of theraputic ultrasound. You did not mention why you had your implants removed or if you are open to having them replaced with non textured implants, as this would also be a way to correct the problem. If conservative measures have not achieved the desired result after three months , some type of surgical correction will probably be required. This might be as simple as release of scar tissue or involve more complex procedures with the use of tissue substitutes such as Alloderm .

This should improve with time and massage.  Based on your picture this can probably be fixed with fat transfer to the breast. It is not unreasonable to observe a waiting period of six months after explantation before the determination for breast lift or breast fat transfer is made. This will give your breasts time to bounce back and your PS time to determine what type of lift, if any, is needed. An in-person consultation with a board certified PS would be best to determine the optimal time for the fat transfer.

In your frontal view, your breasts are hanging low on the chest wall.  I recommend a lift using The Bellesoma Method.  This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage.  Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained, if not injured during your previous surgery.  At the same time or later, fat transfers can be performed if additional volume is desired.Best Wishes,Gary Horndeski, M.D.

The dents are due to scar tissue adhering in your breasts.  This may be correctable with massage and time, although after 3-6 months if the problem persists, then correction with fat grafting and/or a smooth implant may be indicated.

I understand your concern and it is quite normal to be concerned about the outcome following implant removal surgery. However, you are still in the very early stages of the healing process.Your will need to be patient at this moment and follow what your surgeon has instructed you to do post operatively.At the early stage of the healing process, the tissues are still swollen. Once the swelling subsides  you will be able to see the final result of your surgery.

Thank you for sharing your question and photographs.  Based on the 5 week history of explant your body has produced a scar response as part of normal healing.  These adhesions, and any tissue firmness, is likely to improve spontaneously with additional maturation.  I would recommend scar massage to the tissues and area of adherence, and revisit your results at 3-6 months after treatment.  Hope this helps.

corall123I appreciate your concerns and frustration after removal of your textured breast implants. It appears from your photographs that you would benefit from either replacement with smooth implants or fat transfer to both breasts to fill out the depleted, deflated areas and also correct adhesions. Hello and thank you for your question and photo's.  I would wait 2-3 months before having anything done .   I recommend a round smooth implant with a lift .  Your skin has been stretched out and will stay that way after all is said and done.  You can also just have a lift without implant as well.
