Is second Breast Lift safe 3 yrs after? And can weight gain and loss of 10 pounds cause breasts to be empty again? (photos)

The short answer to your question is yes, it is safe to do re-operative surgery; however, it is important to know as much about the first surgery as possible in order to "provide a road map" for your next surgery. Once your re-operative surgeon knows the approach that was taken before, they can then devise a safe route to make sure the circulation remains intact and you get your desired outcome. As far as weight gain and loss, I would say that it is quite similar to pregnancy weight changes that can cause deflation in the breast tissue; But given that we know your skin tends to give way to the drop in breast tissue it is best to provide internal support to help prevent recurrence of this problem. breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery.Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size.While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. An uplift after 3 years sounds as safe as it will ever be as long as you are in good health. If you are happy with the shape of your breast it is possible to reshape your breast tissue in a way do that it gives you better definition in the cleavage area and fuller upper pole

You might want to consider minimally invasive radiofrequency skin tightening to tighten that small amount and avoid surgery.

From the photo it appears you would benefit from a 2nd breast lift. It's unlikely you'll maintain upper pole fullness without an implant as you need more volume. Weight gain and loss affect women differently but generally speaking a ten pound gain or loss does not produce a significant difference in the breasts.

You have several things going on.
First, you lack volume (so yes, a breast implant would solve this issue)
Second, you have excess skin. This is reduced with a second breast lift however, you need some volume underneath to help round out the breasts and you are on the borderline (low breast volume). An implant helps this too of course.
Third, if you have some extra fat, liposuction with fat transfer to the breasts does aid in filling the pocket somewhat, but generally the maximum size increase is 1/2 cup size (some say 1 cup size but that is more rare).

From your photograph it seems that you might benefit more from bilateral breast fat grafting which will result in improved fullness and correct asymmetry of your breast. This recommendation is made since your breast and nipple positions are not low but they look deflated only. Lifting them up will not correct this problem. Hi and welcome to our forum!From your photos, I note loss of superior pole volume and the presence of significant stretch marks about the breasts.Stretch marks indicate the presence of permanently damaged overstretched skin usually occurring during pregnancy or significant weight fluctuation where when pregnant or when overweight, the skin's dermal elastic fibers rupture, resulting in the loss of elasticity and the development of streaks on the skin surface. The skin will stretch over a greater surface area and will predispose to further sagging. Fluctuation of 10 pounds up or down may slightly affect the amount of recurrent sag.It is safe to undergo secondary mastopexy. 15% of cosmetic procedures benefit from a secondary touchup.
