I would recommend performing an implant with a lift or just a lift. You are not a candidate for an implant alone.

From your profile photo, it appears that your nipples are located in line with your breast fold. Although this only shows your left breast. Your right nipple may be lower. Breast implants alone may be sufficient to give you a subtle lift. However, the weight of the implants may accelerate the rate of sagging so it would be a good idea to choose proportionately sized implants. A breast lift may be recommended if you want much perkier breasts or if the right nipple is lower than your breast fold.

The technique I recommend is a lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired.

I recommend an in person consultation with a plastic surgeon to discuss your goals and be measured. After I measure and discuss goals with my patients, we then look at pictures of women (my patients) with similar measurements with different size and style breast implants. They also get to see what they would look like when the breasts have fully dropped and fluffed and in clothes. You get to see actual results and the surgeon's experience and not a virtual idea of what can be achieved. Without an examination it is hard to say for sure, but it looks like you could do with a breast augmentation by itself. The right implant selection would give you better upper pole fullness and shape without too much volume. The best thing to do is to have an in- person consultation or two with board certified plastic surgeons and see what they recommend. I hope this helps.

The implant will help fill out your upper cleavage, increase size and provide some minor elevation of the nipple. It's the easiest procedure with least scar.
If you are interested in increasing perkiness then you need a lift to narrow the breast. You may need only a lollipop shaped incision for that.

If you are happy with your size and appearance in a regular bra without fillers or push up, just do a lift. Any implant will make you at least a cup larger based on your breast width.
