Examining your photographs, I see some asymmetries in size, shape, and nipple positions as well as some emptiness in your upper poles and drooping of both breasts. In my opinion, the best way to address all of these issues is with a breast lift and fat grafting. The breast lift will address the shape, drooping, and nipple positions. The fat grafting fills the upper pole, adds relatively more fat to the smaller breast, and better shapes both breasts. Both procedures have their advantages and limitations. The mastopexy/lift requires additional scars on the breasts, and the fat grafting is limited by how much fat can be placed into each breast.

my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity.

It is unlikely that you will accept the aesthetic appearance after a lift alone. You would have to either compromise your goals or accept a small implant. Fat grafting could be an option, but it does not appear that you have enough donor fat available.

The technique I recommend is a breast lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired.

Thank you for your pictures and questions. If you want additional volume and/or shaping of the breast then fat grafting is really your only option. However, you do not seem to have much fat to donate to a project like that, so in that case something like Alloclae might be an option for you but it can be pricey. As for the breast position, to change the position and reduce the skin envelope, you would need some type of lift (mastopexy) to achieve your goals. Hope this helps!

In order to achieve the look you are wanting a breast lift with small implants would be recommended. The implants do not need to be large, but will be helpful in shaping the breast and providing the upper pole fullness you are wanting.
