What can I do to have less skin/fat and a nicer side boob? I had a breast lift / saline implants 2007. At the time I complain that I felt I had too much fat on my side boob area and dr extended the scar. Unfortunately all that did was provide a lip for all of the skin and fat to hangover and I feel like my boobs are boxy. I am looking to have an arm lift, breast revision and want the best options to include this area to have a tight chest wall/ pretty side boob. 145 lbs, 5'3", lost 70 lbs.
Answer: Revision Augmentation/Lift Your implants may need to be replaced based on physical examination and high resolution ultrasound. In addition, you will need a revision breast lift. Your lateral chest wall will need a skin excision leaving you with an extension of the inframammary scar out to the side of your chest. If you choose, you could be a good candidate for an arm lift as well.

Answer: Brachioplasty and lateral chest wall contouring usually require trading improvement in contour for scarring As it seems you are very aware of, the lateral skin under your armpit is not exactly your breast tissue but is more your lateral chest tissue. These are aesthetically different structures as well as functionally different structures. Some excess skin in the lateral chest wall and in the arm are required for proper movement of skin around the body during normal day-to-day activities. When we as plastic surgeons perform procedures on this lateral chest skin and arms , we must be careful to explain to our patients that scars in these areas often widen due to the amount of movement in these areas. It is likely that a skilled plastic surgeon could perform An arm lifting procedure that includes an incision on your chest which may help improve his contour. Most of these incisions heal relatively well however the scars can be widened and this is important to note.

The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. In my opinion a combination of liposuction and direct skin excision will help contour the side of the breast and your upper flank area. This is common in patients who have had weight loss such as yourself and a board certified plastic surgeon should be able to help answer these questions in consultation.
