I'm 250/270cc moderate rn and exchanging for bigger because I want more upper pole. My surgeon is deciding with moderate plus and 350 and 385. I don't want to be too much bigger.. just better upper pole. This won't make me MUCH bigger right? I'm also getting pocket tightening and galaflex from bottoming out/lateral displacement

Answer: Don't Biggest trap in the world: thinking that more upper fullness will be achieved with a larger implant without problems later. Even if you get the upper pole fullness that you seek (amd the implant increase you mention is significant), your breasts will eventually stretch again and you will get accentuation of the bottoming out deformity that you already have in the photos.

Answer: Volume differences 100cc difference should change your overall volume by less than a cup. Upper pole fullness is largely determined by implant projection and implant positioning. Reinforcing your IMF and adding mesh will all for the implants to sit higher allowing for more upper pole fullness. Ultimate implant selection should be based on your native breast base width and match this up with the appropriate volume and projection. Sean Fisher Aesthetic Plastic Surgeon
