I've had 325 cc mentor smooth gummy implants for roughly 15 years. My surgical pocket had not reduced in size. I can push them to my collar bone easily. Is this a concern? They have become increasingly painful (under muscle)

I would not go larger for your tissue will thin and stretch more and be more prone to sag. The internal mesh bra can give your tissue more support to prevent sagging. Your tissue, initially, was sagging and thin sagging stretched tissue never holds well long term.
You need to make sure the Hidradenitis is completely calmed down before considering any breast revision. The axilla is next door to your breasts and excessive bacteria from a flair up could cause a breast infection. If need, get your Hidradenitis treated first.
You are still early in your healing phase and I would suggest external ultrasound treatments along with the Accolate twice a day to see if your pocket could soften and let the implant settle down lower.
