Would I benefit from a breast fat transfer?

Fat transfers will provide a modest increase in volume as well as some improvement in shape and contour. Removing the side flaps will improve the contour but it will extend the scar towards your back

This is a good case for VECTRA imaging, in my opinion. The machine can take high quality images which can then be 'morphed' with different mastopexy techniques. It's not perfect but it can give you a rough idea of the effects of different types of 'lifts'. In my practice, this has been very helpful for patients trying to make a decision.

Since you had contracture, an implant is not a good option. Fat transfer is a great option if you want more volume and shape. Not nearly as complicated as implants and will be a durable, care free option. Thank you.

Based on the pictures you have submitted I would say you would benefit greatly from fat transfer to breast in addition to some removal of skin from the side of the breast. I think this would contour your abdomen nicely and give you an size augmentation of the breast that would be appropriate for your body type. Removing the skin from the side of the breast will improve the contour and adding the fat will give additional volume.

Fat transfer is a great technique to improve breast shape and add just a small touch of volume and is ideal for patients who do not want a significant increase in size. This procedure can be combined with liposuction +/- skin tightening (Renuvion) of the lateral chest (as an alternative to excision of "the side flaps") to improve the shape of your breasts. Fat transfer over the implants, or remove implants and replace with fat? Both could be done. Agree with your surgeon, liposuction and skin removal to also enhance breast shape.
