10 months post Breast Lift and Fat Transfer - Why is my areola/nipple puffy? Should I be concerned? (photos)

I am 10 months post breast lift and breast fat transfer and I am noticing that inside one of my Areola right around my nipple is puffy and some days are worse then others. Sometimes I feel slight sensitivity but otherwise not much discomfort. I am wondering what this would be from and if I should be concerned?
