10 months post Breast Lift and Fat Transfer - Why is my areola/nipple puffy? Should I be concerned? (photos)

Usually the swelling coming and going fades after 6-9 months- bit it appears that your surgeon left your areolas very large- I think an areola reduction will solve the issues!

I understand your concern. However, without a proper assessment, it would be difficult to determine what went wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice

This could be from the fat transfer, but I would suggest going back to your surgeon and being examined.

Preoperative photos would be beneficial in evaluating your issue.
First, there is nothing wrong, I would assume that the increased pressure from tissue below after your lift is causing the area to be puffy. A physical exam would aid in diagnosis.
