I am looking to get a breast lift possibly one with minimal scarring along with either a fat transfer or a small implant to enhance fullness in the upper part of the breast. I have had 2 kids both breastfed and am 158 cm and weigh 49 kgs. What are my options?
Answer: Surgical scars can be minimized by following proper after care treatment with lasers, creams, and injections. Plastic surgery scars require attention after surgery so that they remain cosmetically-appealing and barely noticeable. In our office, we use a protocol for scars that minimizes their activity in the 6-8 weeks after surgery. If you have late plastic surgery scars, you may also require laser and RF therapy to help reduce the longterm effects of the scarring. Keloids, hypertrophic scars, widened scars, red scars, and hyperpigmented scars all require scar modulation with topical creams followed by lasers and RF therapy. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired. I would suggest meeting with a board certified plastic surgeon, being measured and examined, and discussing your options. During the consultation, you can try on a variety of implant sizes to see what you might like. Implants behind the muscle will give nice upper breast fullness. Answer: Breast lift Dear bg1995, my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations. From your photos, I note minimal sag with the nipples residing well above the level of the fold at the bottom of the breast. However, the areolae are moderately enlarged. One is tempted to offer a Benelli periareolar lift (donut lift), but due to areolar size and the amount of lift required to overcome it, the breast shape may pancake (flatten and broaden). You may consider a lollipop (vertical) lift which will reduce areolar size, place the nipple-areolar complexes in anatomic position, and conify the shape of the breasts. Breast augmentation with either implant or fat can be performed at the same time. Visit a board certified plastic surgeon for evaluation, examination, and discussion. Best wishes...
