Hi, thank you for sharing your question. You are a great candidate for breast augmentation. You will definitely have upper pole fullness. Based on your photos, you will likely not require a lift. It's a little challenging to tell from these photos alone. By the appearance, you can certainly achieve upper pole fullness without a lift, but because of the position of your breast, it would appear that your breast had "fallen off" the implant, hence the term "Waterfall" deformity or "Snoopy Nose" deformity. Your breast would still have the same "droop," and would not be centered over the implant. Some women don't care about that because they will wear a bra at all times. But if you're looking for youthful, perky breasts with upper pole fullness, I recommend a lift.
Because your nipples appear to be located below your breast fold, a lift is recommended if you’d like more upper pole fullness. Without one, the implants would sit rather low on your chest and their weight will most likely exacerbate the sagging more, making a lift inevitable in the future.

Its a common question, the issue with upper pole fullness without a lift is that the implant can sit higher up and the breast tissue will sit over the implant and can appear to be sagging or what we call "snoopy deformity". A full consultation and examination at your Plastic Surgical office maybe in your best interest to get the results you are looking for.
Your breasts are very low on the chest wall. Adding implants will results in large low breasts. The technique I recommend is a lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. The weight of the breast is transferred to the underlying pectoralis major muscle resulting in pain relief and long term stability. Good afternoon,You can but only with a very large implant (650-800cc?) so I'm not sure if you want or are comfortable with that size- otherwise you may need a lift if you want smaller implants!
Your breasts are large and hanging well below the fold. An implant will give you all the upper pole fullness you desire, but your breasts will not look normal or attractive. Your natural breast tissue will hang off of the implants like water running off a cliff at the top of a waterfall, hence the term 'waterfall deformity'. Conversely, your implants may slide down the soft tissue envelope creating very saggy, very bottom heavy breast with little upper pole fullness. If you want beautiful, objectively attractive breasts, get the breast lift. The scars will become inconspicuous.
Without an examination in person it is difficult to render a specific opinion or recommendation, however based on your photos, if the degree of ptosis (natural drooping) that you currently have is not a problem for you then a subfascial augmentation (not sub pectoral or sub muscular) with more than likely a Moderate (Classic) profile implant of adequate width may achieve the desired outcome. Your breasts will be heavier and there is a risk of further droopiness which will indeed ultimately be best corrected with a mastopexy (breast lift). Alternatively, you appear to be a good candidate for a mastopexy currently, possibly without the need for an implant at all. Please consult with an experienced board certified plastic surgeon whose aesthetics seem to align with your own based on his or her Before and After gallery.
