We always recommend a 2/1 ratio for these areas to help balance both areas. If you are just treating the forehead lines this may cause your eyebrows to droop. There are several ways we can treat the muscles around the brows to lift eyebrows1. Treating crow's feet (by relaxing contraction, we can have a more lifted brow)2. Treating 11s (can lift central brow)3. Treating UPPER half of foreheadI specifically say treating upper half of forehead because the forehead can be split into lower forehead and upper forehead by something that we call the 'line of convergence.' When we are treated above the line of convergence (upper forehead) we can often create a brow lift. However, if treated below this line, we risk dropping the eyebrows.
It may be possible for you to get a lift from having tox placed in the upper forehead, however as we age and the skin in the forehead has less support (volume loss from temples, skin laxity, etc) then the risk for brows lowering after treatment becomes higher, even if we are treating above this line of convergence.
Sometimes, based on a patient's anatomy and degree of eye-hooding, we may completely recommend against treating the frontalis. However, you still may be able to achieve a subtle brow lift from treating the crow's feet and 11s.

The frontalis muscle is the only muscle that lifts the brows. If it is relaxed a little, then the brows will drop a little. If it is relaxed significantly, then the brows will drop significantly. Other muscles, which pull the brow down, can be relaxed (the brows and crow's feet) and this will allow the frontalis muscle to work without opposition. This is what lifts the brows. If you want maximum brow lift from botox, then only the crow's feet and brows should be treated. This allows the forehead muscle to lift the brows and it causes forehead wrinkles. If you get botox in the forehead, then the brow lift will either decrease (low dose forehead botox) or cause brow droop (normal dose forehead botox).

As we age, our eyebrows and upper eyelids descends, as our eyeball moves posteriorly. The frontalis muscle is an elevator, so if it gets paralyzed, it has a more difficult time to elevate the eyebrows and eyelids. As a result, it's a balancing act to make sure the neuromodulator (generic term for botox) is placed correctly with the right dilution to minimize lower forehead heaviness while relaxing the frontalis muscle. The tails of the eyebrows can be lifted with neuromodulator, but sometimes there can be wrinkles that develop upon raising the eyebrow. However, if the eyebrow descent has been severe, there are other solutions such as lasers to help elevate the eyebrows. Fractionated CO2 laser can help relax the wrinkles on the full face.
