Hello, a nurse injector at my dermatologist recommended under eye Botox for creases when I smile. Effect has been terrible, with swelling and a massive wrinkle above cheek bone when I smile. I’ve also now noticed bulges and elongation of my tear trough when not smiling. Is this fluid or fat herniation (something else?). Will it resolve on its own? If so, how long will it take? I don’t recognize my face in the mirror any more and it is very hard. Thank you SO much.

Answer: The lower eyelid is not really a good location for Botox. This is not swelling. This is weakened lower eyelid muscle. Your muscle is paralyzed and slumped causing the tissue to heap in this unattractive fashion. I recommend not chasing this problem. Eventually the Botox will wear off and hopefully this issue will resolve. Specially, do not well meaning providers laser or inject this, you will end up with a permanent and hard to fix problem. This can take 4 to 6 months to resolve. Also find a new injector,

Answer: Under eye Botox Under eye Botox can be tricky. It can help with extra thickness of the muscle around the eye itself. It does this over time by allowing the muscle to thin out since it is not contracting when it is relaxed. The flip side to relaxing the muscle (orbicularis oculi) is that it is no longer got strong tone which keeps it tight. Imagine the muscle like an elastic waistband. If it relaxes, then it is loose. Because it is loose, the tissues behind it can push forward while it is relaxed. Botox is temporary and usually wears off in about 3-4 months. The muscle will regain its tone and there will be less bulging. It is reasonable to get a lower Botox dose in the future which will allow a little relaxation of the muscle without fully relaxing it. Another option is to avoid Botox in the under-eye region altogether while accepting the muscle will maintain it's thickness and cause wrinkling.
