Hello, About 12 years ago I had a breast lift and augmentation to correct my small tubular breasts. They were gel under the muscle. The results have been wonderful, however, I would like to have them removed, but I do not want implants again. I want my breasts to look good post explant without getting new implants. What are my options? If I choose to do a lift, will my breasts be tiny as hell because they were tiny pre-augmentation? Would getting a lift with fat transfer be my only option?

Answer: Explant and Lift It is very difficult to answer your questions without photos and a proper exam as well. But in general, some patients will need a lift after explant, some will not. Some patients benefit from fat transfer, in particular to the upper pole of the breast to help fill it some of the hollowing that is evident with implant removal. I would recommend you see a board certified plastic surgeon that has a great deal of experience in revision surgery. Breast revision surgery is significantly more difficult that primary breast surgery. Hope that helps.

Answer: Explant with Bellesoma Method The technique I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.
