Hello. I have motiva 825 cc corse (uhp) implants. (Before that I had 450 cc hp silicone) They continue to bottoming out and now again for the 3. Time. I am going to have smaller implants and textured. I think about mentor hp 600 cc (14 dm and 5,7 proj) or mentor xtra high 680 cc(14 dm and 6,6 proj) I am 175 cm and 63 kg. I do not want a breast lift. I would like the upper fullness from the xtra high but are afraid that 680 is still to heavy to fix me from bottoming out? Want a small cleavage.
Answer: Revision surgery Dear chicka23, Breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery. Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size. While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras. If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed. Only after a thorough examination you will get more information and recommendations. Also, you have to make sure that your surgeon understands your breast augmentation goals. Answer: Size You are thin and have minimal breast tissue. You cannot support large implants. Even with your first photo, you are bottoming out. You need to downsize to around 350 cc's and use mesh. Heavier implants will still drop. Plus, large round balls on your chest are hard and not pretty. Your implants should also be placed behind the muscle for more support. Most women cannot and should not try to support large heavy implants.

Answer: Explant with Bellesoma Method Your breasts are wider than your chest wall, low on the chest wall and mechanically unstable. I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, small implants or fat transfers can be performed if additional volume is desired. Best Wishes, Gary Horndeski, M.D.

Answer: Bottoming out and downsizing again Thank you for your inquiry! Larger implants are heavier and can settle lower. Internal scaffolding would help decrease the risk of bottoming out. We would love to sit down with you and discuss your concerns. Schedule a consultation with a Board-Certified Plastic Surgeon to help you reach your goals.

Answer: Bottoming out and downsizing for the third time You have the typical problems seen with implants that are too large . I prefer smaller implants. Smaller implants have fewer problems and look more natural
