I had upper and lower blepharoplasty over a year ago to treat mild creases under my eyes and excess skin on my eyelids. I appear to have dark bruised like skin under my eyes, I’m particular under my right eye that I didn’t have before. How can I get rid of it? Is it safe to have another lower bleph? I already have rounded corners and lower eyelid retraction on my left eye. I have very large eyes and flat cheeks.

Thank you for submitting your question and photos. You may benefit from a facial fat transfer to add volume to the sunken areas beneath the eyes and reduce the dark appearance you’re referring to. Harvesting fat from the abdomen or thighs and meticulously injecting it where needed can rebuild volume and create a natural, supple contour while improving the quality of the skin. Another option might be a bit of dermal filler injected into this area. I recommend scheduling an in-person consultation with an experienced, board-certified facial plastic surgeon. They will be able to review your surgical history and determine the best procedure to help you achieve your aesthetic goals.

At home peels like cosmelan, enlighten and aerify along with dermarolling (MTS roller with anteage MD ampules) should be started now. There are many things you can do to treat under eye hollows and dark circles. A surgical option to remove skin and/or fat will such as lower eyelid blepharoplasty with or without fat pad repositioning will be the best long term. There are many things you can do to treat under eye hollows and dark circles without surgery. Non invasive less downtime options such as fillers (Belotero, Juvederm Volbella​), lasers (Moxi/Halo/Fraxel/CO2​), Renuvion (J-Plasma), microneedling/PRP, stem cells and microneedling RF like Fractora PiXel8 or Morpheus can all help give some improvement. Accutite can often be used or stronger lasers like erbium or co2 spot treatment can tighten the skin and give very quick improvement in wrinkles and texture. Thankyou for your question! I have had a lot of success with under eye hollows and darkness using nano-fat grafting, even in post operative situations. You would, however, need a careful analysis of the cause of your persistent under eye darkness. Please discuss this with your surgeon who can analyse your problem and help you with your concerns. You may want to consider adding a little filler to help give volume to the under eye and cheek area.  Sometimes the dark circles are really just shadowing due to loss of volume.  See an expert injector near you to discuss if this treatment is right for you.
