Keloids on lipo ports have now got white patches due to kenacort injections. Any suggestions?

Hi.I got a multiple area major liposuction surgery done on 6th May 2015.All my ports got/became keloids & the doctor administered kenacort injections thrice with a gap of 3 weeks in btwn every set of injections.Now I have reduced keloids HOWEVER they've got visible white patches around them please advise as to 1.why this occurrence of white patches post kenacort 2.Are these permanent 3.how can one restore skin colour in those patches 4.CAN THIS BE THE ONSET OF VITILIGO?!? I AM EXTREMELY WORRIED

I am sorry that you are having concerns about your procedure
and thanks for sharing your question. I can appreciate your
concern.

1.why this occurrence of white patches
post kenacort? The corticoids affect the melanin (skin pigment) giving the lack
of pigmentation.

2.Are these permanent? In the majority
of cases yes. You need to wait several months before seeing the final outcome.
Make sure that there is a dermatologist involve in your care for the
hypopigmentation.

3.how can one restore skin colour in
those patches? Sometimes sun exposure might help. However clear this maneuver
with your plastic surgeon or dermatologist

4.CAN THIS BE THE ONSET OF VITILIGO? It is
localized and secondary to the corticoids. Vitiligo usually is autoimmune.

Finally, make sure that your doctor is
aware of your process. He should be able to guide you best in your care.

This is a common occurrence after undergoing any intralesional steroid injection. In most instances, this is a permanent sequela of the steroid injection. In this instance the steroid used was Kenacort. Conservative measures to restore skin color include the use of Retin-A and hydroquinone and some laser treatments to try and reduce the size of the white patch. Also tattooing can be an effective means of matching the color. It is very unlikely your vitiligo is reoccurring in these areas where you had the injections. Best wishes.
