I did two sessions of kybella last year and then also lost 45 pounds. My chin area has fat and a little loose skin. Hopefully I can upload photos. I’m looking into chin liposuction but my question is, will that help or will I also need some type of skin tightening as well? I was hoping not to go the neck lift route. Have been told about procedures such as facetite and renuvion but am worried they don’t work for everyone?

The double chin is accentuated by a very deep mentolabial crease. In order to get a nice result without the necklift, I would recommend some degree of liposuction with a platysmaplasty and moving a portion of the deep fat into the crease to give you a smooth neckline and eliminate that crease. This is called a pedicled flap. It works nicely in cases with a deep crease. In addition, any type of skin tightening procedure with Facetite or Renuvion would be beneficial.

If you pinch the area under your chin from jawbone to jawbone, is it skin on skin, or can you pinch an inch of fat? Neck liposuction after previous Kybella is a bit challenging due to scar issue, but this may be considered if there is excess fat. There are two things that should be addressed surgically: 1. Laxity in the skin: I think a neck lift would be a better option as the extra skin can be re draped. Liposuction, even laser liposuction, will leave you with a lot of loose skin. 2. Deep neck fat the equivalent of visceral fat in the belly: There is fat that is deep to the platysma muscle that can't be reached with liposuction and Kybella.
