1. I would like to remove the fat around my jaws and under my chin followed by a facelift. I’m not interested in doing injections or devices (CoolSculpting, ...etc.). Am I good candidate for liposuction followed by facelift? 2. I’m concerned about the sagging and hollowness under my eyes. I’m not interested in doing dermal fillers or fat grafting under the eyes. I’m wondering if a mid face lift, cheek lift, or blepharoplasty will help? maybe combined with fat/ tissue repositioning?
Answer: Facelift/Necklift w/ Submental/Neck Liposuction Hello and thank you for your question. From what I can tell based on the pictures you have provided, there is obvious fullness to your chin/submental region into the neck, and loss of jaw definition, as you had mentioned. You do not appear to have visible platysmal banding in your neck. Based on all that, you would definitely benefit from facelift/necklift with liposuction to your submental/neck region. This would be the best way to address the fullness you're concerned about, and restore a more youthful appearance to your face and neck. At the same time, facial fat grafting can also be done to help further correct any asymmetries or restore volume to focal areas of concern. Regarding your eyes- I see what you are referring to, however, I would not recommend any of the surgeries you asked about in your case. If there was instead some bulging of the lower lids ("bags under eyes") which commonly occurs with age, then you would be a good candidate for a lower lid blepharoplasty, but that is not present in your case, so this would not be indicated. The only thing I would recommend which might help soften the transition from lower eyelid to cheek would be facial fat grafting, done at the same time as the face/neck lift as mentioned above. If you do not wish to address the eyes at this point in time though, they can always be addressed later or if you start to develop the bulging I mentioned. With all that said, it is always difficult to provide tentative surgical recommendations, as there is only so much information to be gained from photos. I recommend for you to come see me or another plastic surgeon for a formal consultation to thoroughly discuss your surgical goals, undergo a formal examination, evaluate all your options (surgical and nonsurgical), and decide on the best procedure(s) for your specific case. Answer: Face I would suggest liposuction of your neck with minimally invasive radiofrequency skin tightening like Renuvion, plus micro-needling and radiofrequency for the rest of your face.

Answer: Deep plane necklift and facelift The best option for addressing the fullness in your neck would be a deep plane necklift, together with some liposuction. You are unlikely to be happy if just liposuction is performed without addressing the deep structures. This could be combined either with an extended deep plane necklift or a deep plane facelift to address the resulting skin excess. For dealing with the hollowing under the eyes, my preference would still be to start with fat grafting, but importantly not an excessive amount to prevent making you look feminine. A midface lift is an option, but I find it doesn't work quite as well.

Answer: Neck and lower lid solution Thanks for including these excellent photos with your question as it allows us to really see what your concerns are and how to best addressed them. If you want a result that looks as natural as possible, my recommendation would be doing an extended deep plane facelift. This would do two things that would address your concerns. 1) this is the only type of deep plane facelift that releases the zygomatic ligament, which teathers the mid face and prevents rejuvenation of the lower lid cheek junction. When those ligaments are released, the sagging tissue from the area around the mouth and lower cheek is able to move up and fill the lower lid cheek junction. Unfortunately, you do not have lower eyelid fat herniation that would allow for a successful fat repositioning blepharoplasty. As for your neck if you’re doing a deep plane and you are under the platysma, you very rarely need any liposuction. Liposuction can lead to central submental depression, what we call a Cobra Nexk deformity. Depending on the lipo to thin the under chin and neck area can remove the subdermal fat necessary to maintain a natural look, leading to a thin skin, skeletonized look.
