I am interested in getting rhinoplasty done (preferably a closed rhinoplasty). My main concern is the size and shape of my bulbous tip. I have a very prominent bump on the sides of my nose that I would like reduced. Would it be possible to achieve a slightly thinner nose with closed rhinoplasty? (while keeping the overall round shape)
Answer: Closed Could Work, But I Usually Prefer Open Yes, you could get a nice result with a closed rhinoplasty. Addressing the bump and narrowing the nose can be done just as well from a closed approach as an open. However, although it is possible to get a nice result with a closed rhinoplasty, elevating the skin allows much more flexibility in modifying the tip shape without removing too much cartilage (which can result in loss of support and a pinched or asymmetric tip). Although I used to perform exclusively closed rhinoplasties, now I perform more than 90% open. I feel that my results are more reliable with fewer revisions. I see no problems with the scars, and with careful technique, I do not feel that the recovery is significantly longer. However, there are a variety of opinions on this subject. Individual surgeons have their own preferences of how to produce the best results in their hands. What is more important than whether a closed or open approach is utilized is the experience and artistry of the plastic surgeon and your comfort level and confidence in that individual.Following the advice from a surgeon on this or any other website who proposes to tell you exactly what to do based on limited 2 dimensional photos without examining you, physically feeling the tissue, assessing your desired outcome, taking a full medical history, and discussing the pros and cons of the operative procedure would not be in your best interest. I would suggest you find a plastic surgeon with extensive nasal surgery experience who is certified by the American Board of Plastic Surgery and ideally a member of the American Society for Aesthetic Plastic Surgery (ASAPS) or a facial plastic surgeon that you trust and are comfortable with.
