I had a closed rhinoplasty about 13 years ago. It left me with uneven nostrils. I don't mind the size difference, but the difference in shape and obvious "notching" of my right nostril is extremely bothersome. Would a small amount of filler be able to smooth out the notch, and give me a result similar to the second picture which i have edited? Or is another surgery my only option?

Answer: Alar rim asymmetry The reason for your notching in the area of the soft triangle of the nasal alae is due to asymmetry and deficiency of the cartilages of the alar rims and tip. I don't feel that a composite graft will be necessary to correct this issue as it does not appear to be scarred to the extent that the soft tissue in this area couldn't be released. I would attempt an alar rim graft after re-aligning the nasal tip cartilages. I think that a dermal filler would be a quick but very abbreviated fix.

Answer: This will need a surgical procedure. The area where you have the notching is usually made up of only skin and does not have any cartilage support. This makes it difficult to maintain that extra fold of skin once it is contracted. If the skin is loose and mobile a small sliver of cartilage graft in that area may provide the necessary support to correct the notching. Otherwise some advancement of local tissue or bringing in of new tissue will be needed.
