Your issue is expansion of the lower pole of both breasts, with dropout of breast tissue. This is the most common reason for excessive nipple upward displacement and peekaboo nipples. You'll need a revision breast lift to remove excess skin and tissue from the lower poles of your breasts. This will centralize your mound under your nipples, and reduce or eliminate peekaboo nipples.

Your breast lift was unsuccessful as your breasts are still too low on the chest wall. At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers or small implants can be placed if additional volume is desired.

I have seen 100's of cases similar to years who have had excellent outcomes. I have developed a technique That was exactly designed for your issues. It is termed "Reductive Augmentation" and this was published in 2017. It combines an internal bra/capsulorrhaphy procedure, revision mastopexy/lift, and removal of inferior pole breast tissue. Thanks for your question. ​Without examining you it is difficult to be certain but based on the photos it appears as though you've had some wound healing issues leading to irregularity of the areolar borders and widened scars. Whether it's a result of the scars stretching or poor elastic fibres in the skin, it appears as though the implants have stretched the lower pole significantly and now a bra cup can't contain the lower half of the breast as well as the nipples, resulting in them showing as you describe. In order to correct this you'll need a full revision of the lift to correct the excess skin and most likely some galaflex mesh or alloderm to support the implants and prevent the lower pole from stretching out again. Although the solution is relatively straightforward, the actual procedure is not so make sure you see a breast revision expert in your area to get this fixed properly. Hope that helps!
