This is my second rhinoplasty and I’m not very satisfied. My nose bridge and tip are out of proportion. My bridge is too small which makes my tip look longer than it already was. Is it possible and safe to shorten the tip? I’m 25, 128 lbs, 5’4 and I have very thin skin as cartilage on the tip of my nose is very noticeable.

Performing a primary rhinoplasty (meaning: a patient’s first rhinoplasty) is very different from performing revisionary surgery for myriad reasons. Revisionary surgery is complex because there is scar tissue to contend with, as well as the patient’s own anatomical limitations and then the prior-surgeon’s decisions that need to be corrected! It appears that improvement could be made in your case, but an in-person examination would need to first take place to be certain. I would focus on bringing harmony to you by de-projecting the tip and making it aesthetically appropriate and confluent. Keep in mind that with every surgery come risks, so please do your research to find an expert in revision rhinoplasty who has decades of experience.

Answer: Revision rhinoplasty to bring back balance... From your photos, both your tip is too long and your bridge is too flat. You can ideally correct the nose by just bringing the tip in. You have to do that as well as bringing the bridge out some. That will create a much nicer balance while still being able to define the tip. Be sure to see a rhinoplasty expert for consultation.
