I would recommend a potentially secondary facelift/necklift. This will permit sculpting of the deeper tissues such as the fat around the mouth, fat below the neck muscle (platysma) as well as directly tightening the platysma. This is also the best technique to permit removal of any potential excess skin. I would recommend finding a board-certified plastic surgeon (American Board of Plastic Surgery) with extensive experience in facial aesthetic surgery.

I would also consider buccal fat removal at the same time to reduce the bulging of your lower cheeks. A less invasive option would be a series of ellacor (micro-coring) treatments. This is a newer procedure that removes excess skin without making incisions.

A facelift will not correct the folds around the mouth. These occur because of fat loss in the area, so while getting a facelift, it is usually good to undergo fat transfer at the same time. If you want to easily improve those areas, you can always try some fillers in the office as well.

Revising your facelift is an option. Make sure to discuss with your surgeon the reasons you are unhappy. In your case the deeper layer, SMAS, should be addressed in a revision.
