If anyone did know and tells you (confidentially), then they would have to kill you to keep in compliance with HIPAA laws.There are lots of experienced rhinoplasty surgeons in Atlanta, and most of them are perfectly capable of giving you a comparable result. Do your research, and BTW, you may want to avoid Tara Reid's breast surgeon, Lisa Presley's facial surgeon, Jocelyn Wildenstein's surgeon, etc., even if they do rhinoplasty surgery.This was not a truly serious answer, even though I suspect your question was probably a serious one, however misinformed. You should look at training, credentials, online and office photographs at the time of personal consultation, and board certification by the American Board of Plastic Surgery. It would be a violation of HIPAA (privacy) if someone disclosed who the surgeon was. I agree that her nose looks significantly better on the after photo with a more refined nasal tip. Please keep in mind, though, that nasal surgery is not only cosmetic but also has significant functional implications with regards to nasal breathing. It would not surprise me if this patient has nasal obstruction from nasal valve collapse as she appears to have some supra-alar pinching. You should choose your rhinoplasty surgeon wisely and see someone who performs a significant number of surgeries annually, both primary and revision cases, and also understands the importance of nasal breathing. It would be a violation of HIPAA laws for any Surgeon or office staff member to violate a patient's privacy.  We don't know for certain what was performed, but her nose does look great.  I echo the sentiments of Dr. Petro that it is nice for the public to see that a Rhinoplasty surgery can have a very natural appearance.  It is also important to understand the this actress had a great starting point, and her nose has been refined to be harmonious with her face. There are a lot of great Rhinoplasty Surgeons in LA and throughout the country who follow the same philosophy and can achieve great results.

The photo shows a bulbous and drooping tip. Other angles may show more details regarding her dorsum. She probably underwent tip refinement using sutures and something called a cephalic trim. 

Your question has generated some interesting comment.  It was good to hear what Grady Core had to say.  I agree with him completely.  You may also ask who did some of the celebrity cosmetic surgery which had bad results.  Consider   Kanye West's mother's abdominoplasty surgeon who wasn't properly trained but was given exposure on a well know talk show.  That poor woman died of complications.

The media, TV, and even some of the surgeons themselves make a lot of hype that implies that if one practices in that zip code that somehow  they are superior in skill just because of their practice location. In reality celebrities are people just like us and don't want to travel great distances to see their doctors so most of them just go to someone close by. The surgeons in that zip code i am sure are just like any other surgeons elsewhere except their results , both good and bad are more often seen by the rest of the world.In fact , if you attend the leading aesthetic surgey meetings in the world the Beverly Hills surgeons are not well represented among the leaders in the field and with a few exceptions have never been. Overwhelmingly the advances in aesthetic surgery have not come out of Hollywood and innovation and excellence in aesthetic surgery comes from all over the world and even from some surprising places. In fact, your own city of Atlanta has been home to what would be considered one of the most excellent training programs in the history of plastic surgery at Emory Hospital and has spawned many world class plastic surgeons, some of whom have been some of the leading aesthetic plastic surgeons in the world. While this celebrity had a good result , I can assure you that there are surgeons within an easy driving distance from you that can achieve this same result. The Internet unfortunately has created a situation where patients go backwards in terms of choosing their surgeons. The best way to find a surgeon who can do this for you is not through Internet images, online reviews, or slick websites which has become routine. The most reliable way is through word of mouth, training background, experience, and recognized leadership in aesthetic surgery, and peer selected awards( not fake ones like the "Patient's Choice Award). This information is not as easily accessible as clicking on reviews but it is out there. A review is nothing more than an opinion given by a person who is unqualified in the first place to judge the surgical expertise involved to correct what the surgeon was given to deal with. Can you imagine reviewing your brain surgeon ? Would you give him a bad review if he couldn't cure you from a brain tumor at surgery despite bringing years of training , experience, and expertise? Why should plastic surgery be any different when certain patients present us with more significant challenges than others such that a heavy smoker may write a negative review because she didn't heal as well as someone else. The person reading the review has no idea, the person is anonymous, and surgeon has no recourse if it is untrue . The Internet also gives the impression that plastic surgeons are all somehow interchangeable like mechanics and they are not given the respect like other types of surgeons yet our training is longer than all other surgical subspecialties including cardiothoracic and neurosurgery. Granted there are bad results from time to time but sometimes a poor outcome is not reflective of the skill of the surgeon but the challenge he was given or poor patient compliance. So, in summary, do your homework and I am sure an excellent surgeon will be not far away. Look on the website for the American Society of Aesthetic Plastic Surgery and the American Society of Facial Plastic Surgery for rhinoplasty surgeons in your area that are board certified. I wish you all the best. 

I think it would be difficult to figure, unless she would tell you.  I think you can find someone to help you by looking at before & afters and reviewing qualifications  I agree she looks very good and her nose is more proportionate to her face making her look younger & more feminine.it is refreshing to see good celebrity plastic surgery! I think you are looking for a conservative surgeon, and showing him/her the picture of what you like will help them know what you want and they can tell you if that would be reasonable.

Unfortunately, I do not know the physician who performed rhinoplasty on Blake Lively. If you are wanting results like hers, I feel you would benefit greatly from a minimally invasive surgical procedure that has little downtime, no visible incisions, no packing, and no pain.
