I have no upper pole fullness. Is my implants bottomed out or nipple too high? (photos)

I recieved a breast lift and saline breast implants in 2014. Desiring "fake" look of implants, and much upper pole fullness. In every bra i wear my nipples are always above the bra even though the cups fit perfectly. Anything I wear with cleavage I have to be careful because where my cleavage is, also is where my nipples fall out. Im confused as to whats gone wrong and if I could change anything
