Rhinoplasty nostril resizing scar (Photo)

This is a very tricky scar to fix.  Typically the alar base reduction scar should be just a little closer to the midline on the bottom part of the nostril.  If you place it on the ascending portion of the curve, it ends up looking like this.  The more lateral the scar is extended, the more visible it becomes.  Excising the old scar and really everting the edges as you bring them back together seems to be the best way to fix this.  Honestly, you should not expect a perfect result but possibly some improvement.  Filler will do nothing.     

The depressed scar that you have can be fixed by excising the scar and reclosing it appropriately so it lies flush like the opposite side. This procedure can be done in the office under local anesthesia so you would not need another trip to the operating room. The procedure is simple and recovery is quick. 

In my experience this is best corrected with a small z plasty within the existing scar. I would ask your surgeon if he or she agrees and is willing to do this.  It would easily be done under local anesthesia. Filler is unlikely to help a depressed scar like this.  By the way, the problem you have is not uncommon due to the thickness of the skin in this location
