Some physicians use neurotoxins like Botox to paralyze the "depressor septi" muscle at the base of the columella to achieve a temporary tip-lift. In our experience, this nonsurgical approach tends to be offered by physicians who do not perform rhinoplasty, and these treatments may lead to lackluster results - i.e. a waste of time and resources. I recommend getting a formal evaluation with a cosmetic dermatologist to go over your desired results. Facial fillers (voluma, restylane, juvederm) along with Botox can be used to improve the symmetry of the nose and lift the tip. Surgery would be needed to improve nostrils or overall shape. Minor defects of the bridge (dorsum), central nose and tip of the nose can be very precisely and quickly corrected with fillers. Improved definition of the tip, and straightening of the dorsum may be achieved. This may also result in straighter, thinner appearing nose. Reduction of a prominent dorsum or tip requires surgery. Major raising of the tip requires modification or grafting of the cartilage.Various fillers are available Radiesse, Bellafill, autologous fat, Voluma and Lyft are options. A non surgical rhinoplasty performed with filler will help blend the angles of your nose to make the lines appear more smooth as well as give you the appearance of a slightly elevated tip. This can be performed with a hyaluronic acid filler such as VOLUMA which can also be reversed with a hyaluronidase injection in case you do not like the look. A surgical rhinoplasty can achieve a more dramatic effect and is permanent although there is more downtime. Thank you for your question. Depending on the cause of your nasal tip droop (ptosis), there are several nonsurgical options. If your depressor septi nasalis is overactive, 4 to 8 units of a neuromodulator (Botox, Dysport, or Xeomin) is a simple solution. If you need structural support to your columella (the part of your nose that connects from the nasal tip to your upper cutaneous lip), a hyaluronic acid filler with good lifting properties (such as Voluma) may be a great option. You may also be a candidate for nasal tip filler with a "lighter" hyaluronic acid filler to help nonsurgically reshape your nose. Another option is to use a dissolving suture / thread to lift the nasal tip. All of these options have significant risk in the wrong hands. Please make sure you see a board-certified cosmetic dermatologist or facial plastic surgeon who has good knowledge of the anatomy and significant experience with nasal fillers. Thanks for the great question! Fillers can be used to cause a subtle elevation of the nasal tip. They can also be used to give the appearance of a straighter nose and for reducing the appearance of humps. I recommend seeing a physician in your area who has experience injecting noses as they will be able to tell you if they are able to achieve your goals using filler or if you require surgery instead. I do not know of any method to nonsurgically lift the tip. Although fillers can be used to improve nasal dorsum contour, the tip is very stubborn and is difficulty to lift without surgery. Adding filler to the tip tends to make it look too full and amorphous (loss of refined shape).
