Can a bulbous Bifid Nasal tip appearance be improved using a semi-permanent filler or is surgery the only option? (photo)

I recommend getting a formal evaluation with a cosmetic dermatologist to go over your desired results. Facial fillers (voluma, restylane, juvederm) along with Botox can be used to improve the symmetry of the nose and improve the tip. Surgery would be needed to improve nostrils or overall shape. The bifidity that is seen is due to thin nasal skin, and a natural cleft that occurs in between somebody's right and left nasal cartilages. In your particular situation, however, your nasal tip appears to be crooked. If you are looking to achieve a straighter nasal tip appearance, rhinoplasty surgery may indeed be a better option for you. you can fix the bifidity with filler which works very well in that location. you can't fix the size of the tip. Plain old restylane lasts up to 3 years there.  I would strongly discourage you from silicone or aquamid which  can cause severe problems in the future.
