I had a necklift past February 2023. I went to see doctor last week to discount results. He tightene the muscle under my chin, Platysmaplasty, and said I now need filler to complete the look. I have sagging skin still under chin area. I am not happy at all with results. What should I do? Is filler common after a Lower Rhytidectomy?

Answer: Not happy with my results 10 months after surgery. What should I do? Thank you for your question and photos. You did not post preop photos. Based on these photos, you cannot expect to get significant improvement from a limited neck lift. In truth, you needed a full face lift including the neck. A limited undermining, will not allow for better redraping of the tissues and removal of the excess skin. Addressing the neck only in facial rejuvenation, creates facial disharmony many times and makes one older and not younger. I would discourage facial fillers for you. You already have a "boxy" instead of an upside down oval shape to your lower face. Fillers will add more fullness into an area that should be thinner and contoured. I believe, you would need a full facelift with correction of your neck results. Good luck.

I am biased against using fillers in the lower face because augmenting the soft tissues of the jawline is contrary to the slim, tapered appearance of youth. The whole purpose of face- and neck lift surgery is to reduce fullness and soft tissue redundancy--why would you want to add more with fillers? In certain circumstances there may be a logical role for small amounts of dermal filler, but not to inflate visibly hanging skin or to try to camouflage jowls. It will make your face look swollen, or in the best case scenario the fillers temporarily improve your appearance before dissipating.
