I’ve lost a significant amount of weight and always had a “neck roll” of fat that is now a flap of loose skin. My chin area also has loose skin that may have fat still but I’m not sure. Would I need a neck/face lift to address this? II also have pronounced nasolabial folds. I’m 35. Can it be fixed with injectables of some sort or is surgical intervention the best option? I plan on having skin removal in my body as well, can the face/neck surgery be done at the same time?
Answer: Lower Facelift and Necklift The best way to improve this is with a lower facelift and necklift. The less invasive approach will be Sofwave ultrasound tightening of skin. Answer: Treatment of Loose Neck Skin and Muscles after Weight Loss Based on your photos, I think you would benefit most from a standard necklift, probably extending into the lower cheek region to remove and tighten the stretchy neck skin and muscles. I don't think just adding fillers in the cheeks will significantly improve the degree of looseness visible in your photos. I would also address the two prominent muscle bands on the front of your neck, to create a smoother neck contour.

This is the most effective way to address the excess skin laxity, tighten the vertical platysmal muscle bands that are causing the "turkey gobbler" deformity, and to overall tighten the skin. Fillers and other "non-surgical" methods would not be worth your investment as actual skin needs to be excised in order to tighten and sculpt the neck (think of going to tailor to remove excess fabric). Incisions are below the chin and behind/below the ears in order to remove the excess skin. Some fat can be liposuctioned as well in the same setting, but your concerns have less so to do with excess fat and more so excess skin/skin laxity. It is possible to pair this with the body procedure you mentioned, but that is to the discretion of your facial plastic surgeon, as he may also recommend a facelift for full rejuvenation and, if performed, will add more time to your surgery.
