28 female 1m57 and 58kgs. had 360 vaser lipo last december. I didn’t do enough research before undergoing surgery. I wasn’t told about the possible complications etc i am now left with a very lumpy uneven belly. some of it is caused by fibrosis. When i am laying down my belly looks completely flat and normal but standing up it looks horrible. I’m so depressed and scared to undergo revision from fear that the next surgeon would make more mistakes. Can my belly be fixed and how? Thank u
Answer: Lumpy uneven belly after Vaser Lipo Dear Dazzling9089, I would have to examine you but it appears that this is a contour irregularity after liposuction. This happens often when areas the undergo liposuction are not feathered appropriately. The good news is that it can be fixed and I have done so with good results before. While there is always a chance for contour irregularities with liposuction, someone who takes their time and uses the appropriate sized cannulas should give you a superb result.I have all my patients wear compression garments and I use multiple sized cannulas to feather out the result. Answer: The best option for you! Hello There is no fat available to be taken from your abdomen. There are sunken areas and areas with fibrosis. I would suggest either performing Vaser and then leaving the fat without suctioning it. This way, the fibrotic areas will soften, and the sunken areas will be partially filled with emulsified fat. Alternatively, sunken areas can be filled with PLA (Sculptra, Lanluma) injections, which will also promote collagen remodeling in the fibrotic areas.

Answer: Poor Liposuction outcome Your outcome is not related to a complication.Your outcome is a pure representation of how the procedure was performed.VASER is a very aggressive form of Liposuction that can remove a lot of fat in a short amount of time. The device offers no benefit in regards to potential aesthetic outcome, but can be useful for surgeons who want to shorten operating time. Unfortunately, because the aggressive nature of the device when procedures are not done well, the degree of disfigurement can be worse.Improving poorly done Liposuction is very difficult and sometimes not possible. As general statements, the better of a candidate the patient was for Liposuction the more likely they are to have a potentially good outcome, even with revision work guide. Secondly the more aggressive the first year was the less likely revision is to improve the outcome.By far the most difficult situation is when most of the ones removed and the small amount of remaining fat is uneven. That appears to be happening with you.To make quality assessment regarding the outcome of any plastic surgery procedure, we always need to see a complete set of proper before and after pictures. If you don’t have before, and after pictures, then ask your surgeon to forward the ones they took. You seem to have tight skin and I’m guessing you are fairly young.Based on those you should’ve been an ideal candidate for Liposuction. The ability to deliver Liposuction results on a consistent basis is more difficult than most people realize. The number of plastic surgeons who approach mastery of Liposuction is in reality quite small. There are major differences in Liposuction skills among providers. To find the right provider Have multiple in person consultations before selecting a provider and considering scheduling surgery. There’s no correct number of consultations needed to find the right provider. The more consultations you have more likely to find the best provider for your needs. The biggest mistake I see patients make is having only one consultation and then scheduling surgery. When consulting with plastic surgeons, I recommend patients bring a set of facial or body pictures to use as reference.Don’t rely on memory of what your body looks like.During each consultation, ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients who had similar body characteristics to your own.Highly experienced providers should have access to hundreds before and after pictures to choose from. in the hands of the right provider, you should have access to more before and after pictures then you have time to look at during the consultation.Seeing only a handful of results may be insufficient to get a clear understanding of what average results look like in the hands of each provider or how many of these procedures that surgeon has actually performed.VASER does not give better or worse results, but the device can lead to worse disfigurement when used by surgeons who lack sufficient experience or skill. In the hands of the right provider, the device works fine time saving.Revision work is many times more difficult than primary procedures. The importance of provider selection becomes even greater for revision surgery.Any attempt at making the outcome better can also make it worse.Improving poorly Liposuction is usually based on either removing more fat with revision liposuction or adding fat with fat grafting. When the first procedure was overly aggressive, then grafting fat becomes very difficult and may not be possible. The grafted fat needs a sufficient layer of host tissue to graft into in order for the fat graft to survive. Attempting to restore the volume is going to be difficult and may require multiple procedures.Unfortunately, the areas that need volume the most have the least amount of host tissue and this is why grafting fat to correct Liposuction outcomes often fails to give good results.Removing more fat on a patient who was over treated in the first place is a challenging proposition Some improvements are usually possible, but converting the outcome to a quality Aesthetic results may not be possible. Each attempts at revision smaller potential gains.Whether revision work is indicated or not is something you will need to decide.By far the most important variable is provider selection and finding the right plastic surgeon to be working with is going to be very difficult. It may require consulting with quite a few providers. Often patients simply don’t know how to select the right surgeon.I wish I could be more optimistic regarding improving your outcome, but from experience, I know how difficult this can be.The best I can really do is encourage others to be more selective regarding choosing providers.Patients typically don’t do a good job at selecting plastic surgeons.Best,Mats Hagstrom MD
