Thank you for your question. You are an excellent candidate for rhinoplasty. I think rhinoplasty is one of the most beautifying surgeries that we do as plastic surgeons. A large portion of my practice is dedicated to rhinoplasty surgery. It is important to only see plastic surgeons with extensive experience in rhinoplasty, ie perform at least 6 per month and have performed hundreds of cases. Also, critically evaluate on-line galleries to see if their art form resonates with yours. Rhinoplasty is definitely a grey haired plastic surgeon's sport. Best wishes to you.
Your nose and most others can done either open or closed. Most surgeons do open these days because that is how they were taught. But there are plenty of surgeons who still do closed rhinoplasty. I do all mine closed

However, it's necessary to evaluate other views to rule out a septal deviation and if eventually can be corrected through a close septorhinoplasty. A frontal view of smiling and showing your teeth will give us more info, in addition to knowing if you have breathing issues.

Open Rhinoplasty gives better direct vision and is easier for revision surgeries involving the placement of multiple Cartlidge graphs to reconstruct a devastated nose. However the majority of primary Rhinoplasties particularly those involving a dorsal hump, a long nose, an over projecting nose, a bulbous nasal tip, a plunging tip , and any combination of these could easily be performed with either closed or open rhinoplasty. Ultimately it depends on the comfort level of the surgeon performing the procedure which technique he employs. In general surgeons who first learned and grew up on closed rhinoplasty will be more comfortable with this procedure particularly if they have a great deal of experience. Generally younger surgeons who first learned and grew up on open Rhinoplasty will prefer open in Rhinoplasty to closed . So ultimately, in your case the type of surgery, whether open or closed, will depend on the comfort level of the surgeon you choose.

What is the “bump” on the tip of your nose? It appears that what you are seeing is the dome of your lower lateral Cartlidge. In people with strong, thick tip Cartlidge and thin overlying skin the Cartlidge can show through two prominently.
