The more idealized the shape you want, the more tissue needs to be removed and/or the larger the implant is required. Don't let unscrupulous surgeons tell you otherwise. Bigger, high profile implants placed over the muscle will not give you what you want.

Hi, thank you for sharing your question. You are a great candidate for breast lift + implants. This is the best way for you to achieve the look in your wish photos. The other techniques that you mentioned are not ideal for you, in my opinion. I hope this helps.

Breast augmentation or fat grafting with implant on the sagging chest will help improve the deflated bust, but it will not improve the saggy shape. You have to get a mastopexy (breast lift) to get a good shape. But in this case, you cannot avoid the scar. However, I would recommend periareolar augmentation mastopexy as a relatively low-scar method. As a method of breast augmentation with implants and periareolar mastopexy, lifting scars remain only around the circumference of the areola. Reduction is not helpful in solving the problem at all.

Unfortunately, simply adding volume would not achieve your goals. To attain the look in your "wish" photos would require a breast lift in addition to implants.

The technique I recommend is The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired.

Thank you for sharing your question and photographs.  It appears that you would most benefit from implant placement and a lift.  The implant will help restore lost breast volume and shape after pregnancy and weight loss, while the lift will remove excess skin and allow for resizing and repositioning of your nipple into a higher, more youthful, position.  Hope this helps

For my patients, I think an exam in person is the best option because it allows you to assess the skin quality. Sometimes the nipples are low and the skin is very loose, so a mastopexy or breast lift with the augmentation is the best option. The mastopexy does come with a scar but usually that looks much better than an implant that is in good position but the breast tissue cascading low and over the implant. The breast tissue needs to be centralized over the implant in order to look good. Make sure to see a board certified plastic surgeon with experience.

A large portion of my practice is dedicated to breast surgery including breast augmentation, breast lifting, and revision breast surgery. in your case, to get the best result, you would need to have an augmentation with an implant as well as a mastopexy. Over the years, I also developed a procedure termed "Reductive Augmentation" in which the redundant inferior pole breast tissue is also removed to create more upper pole fullness. determining whether you need simply a breast augmentation versus a breast lift can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.

Thank you for your question and photos. Unfortunately a proper treatment recommendation can only be made after evaluating you in person, and only a board certified plastic surgeon whose aesthetics seem to align with your own should do so. That said, although your photos are not adequate or sufficient to render an informed opinion, it appears that you would almost certainly require a breast lift, with our without an implant, to achieve the look you desire, Furthermore, the photos you have submitted of you "wish" results differ quite a bit. For example, the first image of the woman in the mauve bathing suit appears "fake" and augmented, yet several other photos appear natural and beautiful.

Expectations need to be realistic and your breasts are not only "deflated" and smaller than you wish them to be, they also are positioned relatively low on your chest, which is not always something that can be corrected. The final result has to do with a combination of your own anatomy, and establishing proper proportions to achieve an aesthetic balance.
