I am 28 years old with no kids. I gained a lot of weight in my early 20s but have worked hard and lost about 50 lbs and am close to my goal weight. My breasts however have turned saggy, heavy , and deflated. I am young and want my breasts to looks perky, lifted and full! I think they actually make me look bigger. I am however nervous about the scars and it’s very hard me to find lift photos with results that I like. I love the way implants look, can I get the look that’s in my wish pics?
Answer: Breast Augmentation With or Without a Lift Hi newb00bs12311, Determining whether you need simply a breast implant versus a breast lift or both can be somewhat complicated. It depends on a number of factors including skin laxity and current nipple position. Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just breast augmentation depending on the size of the implant the patient wants. I often use an implant when I perform a breast lift because it provides more upper breast fullness after the lift. These two surgeries can be done at the same time and this may be optimal as the risks can be minimized. In addition, upper pole fullness is achieved with appropriate implant selection but more importantly, submuscular placement, and precise pocket dissection. The submuscular placement allows the muscle to cover the top part of the implant which allows the upper pole to appear fuller. Keeping away from the lateral chest during pocket dissection allows the implant to remain higher and prevents it from falling off to the side. You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals. Answer: Fix my granny boobs with breast implants or breast implants with a lift (mastopexy) In my practice, sometimes young moms have a few kids and then lose weight but their breasts make them look really much older. Sometimes, when the skin quality is poor with stretch marks and the breast is deflated with low nipples, a breast lift or mastopexy will need to be done even with breast augmentation. In this case, it is best to see a board certified plastic surgeon with many years of experience and a focus on breast.

Answer: Breast augmentation Hello and thank you for your question. The best advice you can receive is from an in-person consultation. Based on your photographs, the best shape and contour could be achieved with an implant and a breast lift. Your nipple sits below the inframammary fold and without a lift in addition to the implant, this will not have a natural shape. A breast lift with an implant can result in a very nice shape if good technique is used. The size, profile, and shape of the implant is based on your desired breast size/shape, your chest wall measurements, and soft tissue quality. This decision should be based on a detailed discussion with equal input from both you and your surgeon. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results. The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person. Best wishes and good luck. Richard G. Reish, M.D. FACS Harvard-trained plastic surgeon

Answer: . Can I achieve my wish pic look with just implants? There are numerous options: implants alone, implants and a variation of a lift, implants and a crescent elevation of the nipple-areola, implants and a donut mastopexy without the incisions of a full lift. They each produce different degrees of cosmetic improvement. Implants alone do not adequately lift the breast when there is significant sagging, no matter the size or the type, and will not produce a good cosmetic result in a patient who also needs some variation of a lift. Keep in mind that following the advice from a surgeon on this or any other website who proposes to tell you exactly what to do without examining you, physically feeling the tissue, assessing your desired outcome, taking a full medical history, and discussing the pros and cons of each operative procedure would not be in your best interest. I would suggest your plastic surgeon be certified by the American Board of Plastic Surgery and ideally a member of the American Society for Aesthetic Plastic Surgery (ASAPS) that you trust and are comfortable with. You should discuss your concerns with that surgeon in person and be very realistic about your potential cosmetic result. Robert Singer, MD FACS La Jolla, California
