She presented with the telltale sign of pulled earlobes (ie pixie ears) and irregularity under her chin due to her initial procedure years ago. After thorough evaluation, we decided together to perform a revision lower facelift and necklift along with placement of a chin implant to help balance her prominent cheekbones. We shortened her earlobes to make them less pulled looking, removed the scar tissue under her chin (with additional liposuction), placed a medium sized silicone chin implant, and meticulously closed her incisions.

65-74 year old woman treated with Facelift Revision - This patient presented with the complaint of insufficient overall result from her first facelift (performed by another surgeon) approximately one year prior. She had hoped for more improvement in the definition of her jawline, as well as greater tightening of her neck skin and platysma muscle. She complained of odd creasing/webbing at the top part of her ear, as well as a pulling sensation in front of her ear (specifically the cartilage in front of the ear canal). After evaluation, we decided to perform a revision lower facelift and necklift under local anesthesia in our Center. Her results demonstrate overall improved jawline and neck contour, with improvement in the incisions (and pulling effect) around her ears. The procedure time was slightly under two hours, and her recovery was approximately one week.

She told us that she had prior deep plane facelifts which by nature are not able or potent to raise the midface and malar fat pad and therefore correct the jowls. So often patients who have deep plane facelift alone or SMAS facelifts may soon experience residual jowls or recurrent jowls soon after their facelift. She also had a sad facial expression which is caused by her sagging jowls and by the droopy corner of the mouth. In order to correct those midface elevation is required. The most important part of the midface to be elevated is the malar fat Pad. In addition to midface elevation and jawline correction she underwent the following procedures at the same time. Forehead and brows Forehead lift and brow lift mainly the outer part or lateral part. This is also called lateral brow lift or temporal brow lift. Eyes Bilateral upper blepharoplasties Face lifting procedures Midface lift with malar fat pad elevation - Raising and suspending the malar fat pad to create cheek elevation and fullness and also correct her jowls. Raising the malar fat pad also raises the corner of the mouth and helps restore a clean jawline. Furthermore, raising the malar fat pad gives cheek fullness and zygomatic projection which is very attractive. Finally raising the malar fat pad which is the most important part of the midface improves the tear-trough area and also softens and effaces the naso-labial folds. Deep plane SMAS facelift The Deep plane facelift in this patient and in general, helps with the jawline and jowls, the lower face and the neck. It does not help with the midface. Upper lip lift The upper lip lift increases red lip show and improves peri-oral proportions. Fat harvesting and transfer to the lips for lip augmentation Fat was taken from her abdomen and her neck by lipo-suction and then processed and transferred to her lips.
