What is liposculpture?

Liposculpture, also called high-definition liposuction or high-def lipo, is a surgical body contouring procedure that strategically removes fat, in specific zones and at varying depths, to create the appearance of muscle definition.

“Typical areas of the body for liposculpture include the abdominal muscles, to create a six-pack, the chin and neck, to sharpen the jawline, the upper arms, to produce more definition, and the waist, lower back, and thighs, to enhance the appearance of the buttocks,” says Dr. Jon Ver Halen, a board-certified plastic surgeon in Colleyville, Texas.

High-def liposculpture enhances muscle definition by taking more fat from areas with natural shadows and leaving a slightly thicker layer of fat over muscular areas, to create contrast and make the muscles look more prominent.

“Whereas traditional liposuction removes small to medium volumes of fat to give a reduced appearance, it typically doesn’t create shape or contour and isn't able to remove large volumes of fat to enhance muscular contouring and highlight curves and shaping to give enhanced results,” explains Dr. Jason Emer, a board-certified dermatologic surgeon in West Hollywood. “Liposculpture defines the underlying bony architecture and muscular definition, making the procedure more artistry than simply a fat removal technique.”

A liposculpture procedure that creates six pack abs is often called abdominal etching or ab sculpting surgery. To fully sculpt the torso, excess fat is often also removed on the flanks (aka the love handles) and back, a procedure called 360 liposculpture.

Liposculpture can be performed using traditional tumescent liposuction, but most surgeons who specialize in the procedure are turning to energy-assisted liposuction techniques like power-assisted liposuction (PAL) or VASER liposuction, that help break up the fat. This makes it easier to sculpt and remove, with less trauma to the surrounding tissue. “Given that these new technologies have transformed the ability of the provider to become more of a ‘sculptor’ versus a doctor removing fat, larger procedures involving multiple areas can be performed at one time,” says Dr. Emer. These longer, more extensive procedures can create more dramatic results, but they require general anesthesia or IV sedation, which can significantly increase costs.

Like other surgeons who specialize in liposculpture, Dr. Emer frequently pairs the procedure with radiofrequency (RF) skin tightening treatments, such as Renuvion and BodyTite, which he says can produce superior results.

What are the pros and cons of liposculpture?

Pros

Liposculpture can create a chiseled, ultra-fit look that's difficult to achieve with diet and exercise alone.
The fat that’s removed via lipo can be transferred to add volume to the breasts, butt, or face, in a process known as fat grafting. As Dr. Emer points out, fat can even be added over muscles on the arms or calves to create the look of more muscular definition, as well as the penis or labia “for ‘down under’ rejuvenation in both shaping and size, as well as increased stimulation and sensitivity through fat and stem cells viable in harvested fat.”
This body contouring procedure can be combined with complementary procedures like abdominoplasty (tummy tuck), a thigh lift, an arm lift, pec implants, or breast surgery, for a full-body transformation.
Results are permanent if you maintain a stable weight via a healthy diet and exercise.

Cons

Most people need at least one week of recovery time, though you may need two weeks of recovery time (or possibly more) if you combine liposculpture with other plastic surgery procedures, or have multiple, large areas treated at once.
Your surgeon will probably recommend that you wear a compression garment 24/7 for the first two to four weeks, and then at least overnight for the next three to six months, to reduce swelling and help skin contract over its new contours.
It takes about six weeks for most of your swelling to go down and up to one year to see your final results.
While liposculpture is considered safe and side effects are rare, all lipo carries risks like contour irregularities, infection, hematoma (a temporary collection of blood under the skin), seroma (a pocket of fluid that needs to be drained by a physician), and skin compromise. Fibrosis (internal scar tissue formation) and skin color changes are also possible.
Weight gain after aggressive high-def lipo can create an unnatural appearance with exaggerated contours, so a stable weight post-procedure (with a low body fat percentage) is essential to maintaining natural-looking results.
Liposculpture can be more expensive than liposuction, due to the specialized technology, increased time, and level of skill it involves.

RealSelf Tip: For patients with gynecomastia (excess male breast tissue), liposculpture can remove excess fat and contour the chest, but it can’t remove extra skin or glandular tissue, as Dr. Emer explains. An experienced surgeon can assess your situation and determine whether lipo alone will give you the results you want, or if you’ll also need gynecomastia surgery (or another procedure) to give you a fully contoured, defined look.

How much is liposculpture?

Liposculpture costs depend on the surgeon’s level of experience, their practice location, how many areas you’re having treated, the complexity of the procedure, the technology used, and whether you have local anesthetic or general anesthesia.

While its average cost is similar to liposuction, liposculpture can be significantly more expensive. Some surgeons employ a combination of technologies to selectively remove the fat around the musculature, tighten skin, and (often) repurpose the fat to create heightened definition.

Dr. Emer also points out that this exacting process can take much longer, and everyone involved in it must be more highly trained and experienced. He also underscores the importance of an aftercare protocol that reduces complications and ensures the best result.

Who makes a good candidate?

As with conventional lipo, healthy nonsmokers with a BMI under 30 and good skin elasticity may be good candidates for liposculpture.

However, surgeons are more discriminating when selecting patients for high-definition liposuction versus traditional lipo, as patients who are already very fit and committed to a healthy lifestyle will see the best outcomes, both immediately and in the long term.

“High-def allows me to take someone who is really active and make them look like all their hard work is actually doing something,” says Dr. Sieber. “High-def lipo is not for someone who doesn’t work out or eat well, it’s not for someone with a high BMI, it’s not for someone with skin laxity. It’s for a very well-defined category of patients—it’s not a procedure for everyone.”

As with all body-contouring surgeries, liposculpture isn't a weight loss procedure. The goal is not an all-over debulking, but rather a targeted winnowing of discrete pockets of fat. It's important to be at your ideal weight and confident that you'll be able to maintain it, so that your results will be long-lasting.

As Dr. Sieber notes, skin quality is a crucial factor in determining one’s candidacy for liposculpture, as the skin needs to snap back and conform to its new contours once fat is removed, especially in areas of etching. Younger people with minimal sun damage and olive or brown skin tones tend to have the best elasticity.

If your skin has grown lax with age or you’ve lost significant weight, you’ll need to combine high-def lipo with a skin-tightening procedure. Radiofrequency treatments, like Renuvion and BodyTite, can help shrink minimal to moderate amounts of loose skin.

People with more extensive sagging may need some form of skin removal surgery, such as a tummy tuck or arm lift.

What’s the difference between liposculpture and liposuction?

A traditional liposuction procedure simply removes areas of unwanted fat, while liposculpture aims for a more dramatic transformation by strategically removing and leaving excess fat, to create the illusion of muscle tone and help you look more fit.

“High-def lipo is different in that we use some sort of technology, typically ultrasound—a machine such as VASER—to help emulsify the fat, and we’re removing fat in more superficial areas, with the purpose of creating more definition in line with the patient’s anatomy,” explains Dr. David Sieber, a plastic surgeon in San Francisco.

Incorporating some form of energy (versus standard tumescent liposculpture) to break up and remove fat allows for more meticulous outcomes with less trauma. Energy devices may also help shrink up mildly lax skin.

“We use a combination of techniques, depending on what a specific patient needs,” says Dr. Ver Halen. “Most of the time, we use a combination of ultrasound-assisted liposculpture, to help break up the deep fibrous tissue holding fat together; power-assisted liposculpture, to remove the fat itself; and radiofrequency, to help tighten loose skin—particularly on the abdomen, arms, and bra-roll area.”

Because the sculpting needs to be so precise, the cannulas (the tubes used to suck out the fat) used for liposculpture must be varied in size, with both larger and smaller gauges in the mix.

What happens during a liposculpture procedure?

High-def lipo usually takes two to four hours but 360 liposculpture to contour the entire torso (including the abdomen, flanks, and back) can take longer, depending on the extent of your treatment plan.

For small areas, this outpatient procedure can be done under local anesthesia with sedation to help you relax. If you’re doing a larger area, more than two areas, or fat transfer to another part of the body, the procedure will likely be performed under general anesthesia.

Here’s how a typical liposculpture procedure works.

Once the anesthesia takes effect, your surgeon injects tumescent fluid into the treatment areas. This sterile solution contains local anesthesia (lidocaine), salt, and epinephrine, a drug that constricts blood vessels in the fat.
The solution “balloons” the fat, making it easier to suck out. “This has the effect of numbing as well as lubricating and emulsifying the fat so that it’s able to be removed with the least amount of trauma,” says Dr. Ver Halen. “It also helps reduce bleeding and bruising after the procedure.”
Your surgeon will then make small incisions, insert a cannula, and begin to remove the unwanted fat. Prior to suction, they may pre-treat it with energy (VASER, for instance) to break it up and ease the fat removal process. With VASER lipo, an ultrasonic probe is inserted through a tiny incision and ultrasound energy frees the fat cells from the surrounding tissue, so they’re more readily sucked out with the cannula. Studies show VASER helps with skin retraction and reduces blood loss during surgery (compared to traditional liposuction).
Providers frequently combine liposculpture with a skin tightening procedure, like BodyTite or Renuvion, often during the same session.
After pockets of fat have been removed, incisions are usually left to close on their own. “Sutures are rarely—if ever—needed to close the liposculpture holes,” says Dr. Ver Halen. “They’re only two to three millimeters in size, so they heal like a freckle.”
You’ll be able to go home the same day. You will be very sore and groggy from the anesthesia, so have someone you trust get you home safely.

Question 8

How long does liposculpture recovery take?

Most people need a week or more of downtime after liposculpture.

You’ll have swelling and some bruising, so you'll need to avoid strenuous activities for at least one week following surgery. Typically:

50% of swelling will subside within a week.
All bruising should fade within two to three weeks.
75% of swelling will be gone within six to eight weeks.

Your doctor may advise you to wear a specialized compression garment at all times (except while bathing) for two to four weeks, and then at least overnight for at least three more months, to help bring down the swelling and tighten the skin. “Many patients wear some sort of compression garment or Spanx for about six months, to keep the area nice and tight,” says Dr. Emer.

In his practice, he also insists on a recovery regimen of “lymphatic massage, IV NAD therapy, and hyperbaric oxygen to ensure proper healing and reduce the risk of complications.”

Your surgeon will encourage you to ease back into your usual exercise routine. In most cases, patients can resume light cardio as early as one week after surgery and be back to their usual routine within a month.

When will you see liposculpture results, and how long will they last?

You’ll notice an improvement immediately following surgery. For some patients, the transformation is dramatic.

Near-final body sculpting results will be visible in six to eight weeks, once most of the swelling has resolved, but “the final results won’t be seen for six months to a year,” says Dr. Emer. It can take that long for the remaining swelling to go down and your body to fully heal.

Results are permanent, if you maintain your post-lipo weight. If you gain even five pounds, the fat deposits that remain in the sculpted area will expand. This can create contour irregularities, so it’s important to maintain a healthy lifestyle and diet.

“Results can be significantly enhanced through peptide therapies, hormonal regulation (such as TRT), and internal supplements,” notes Dr. Emer. He also recommends maintaining and building on your results with nonsurgical muscle toning treatments, such as truFlex and Emsculpt, and possibly even nonsurgical fat reduction.

As early as your initial consultation, talk with your surgeon about how they recommend maximizing and prolonging your results. This is an investment, and you’ll be happier in the long run if you have a plan in place to ensure long-term benefits.

What are the risks of liposculpture?

A 2023 study reviewing thousands of high-definition liposculpture cases over an 18-year period noted these potential complications:

Seroma or hematoma
Bruising
Acute anemia
Hyperchromia (excessive pigmentation of the skin)
Wrinkled skin
Wound dehiscence at the cannula incision site
Infection

Because high-def lipo customarily involves the use of thermal energy, there’s a greater chance of seroma formation, which is why many surgeons use drains to prevent these pockets of fluid from accumulating near incision sites.

Energy devices introduce the risk of burns and related scarring and hyperpigmentation. VASER is thought to be safer than laser lipo in this regard, since it generates less heat.

Contour irregularities, like lumps, bumps, and ripples, can happen with all forms of liposuction. With liposculpture, the risk can be greater (especially in inexperienced hands), since surgeons are removing not just deep fat, but superficial fat from right below the dermis. There’s less tissue to hide any irregularities caused by the cannula.

Dr. Emer also notes the risk of fibrosis (internal scar tissue), which he says “requires lymphatic massage, nutritional NAD IV supplementation, and hyperbaric oxygen treatment daily for the first 10-14 days, and then at least twice weekly for up to 3-6 months to ensure proper contours and less risk of long term complications like contour irregularities, indentations, or skin color changes.” Ask your surgeon about their protocol for mitigating the potential aesthetic risks of fibrosis.

Notably, a study looking at the safety of ultrasound-assisted liposuction over a 15-year span discovered the “rate of complications [with VASER] are lower than that of traditional liposuction with equivalent or higher patient satisfaction.”

Are there nonsurgical alternatives to liposculpture?

No other treatment can achieve the same body sculpting results offered by liposculpture. However, you do have other options if your primary goal is reducing small pockets of fat or building muscle.

You'll need an initial series of four treatment sessions, with regular maintenance treatments to keep up your results, but it doesn't require any recovery time. CoolSculpting uses extreme cold to destroy excess fat, which gets flushed out over the next four to six months. It also requires multiple treatment sessions.
